摘要
目的 探析在甲状腺癌手术患者中实施基于加速康复外科(ERAS)理念的全程化护理管理的效果。方法 回顾分析福建省龙岩市第一医院于2019年1月至2021年12月收治的200例甲状腺癌手术患者的临床资料,将实施常规护理的102例患者归入对照组,采用基于ERAS理念的全程化护理管理的98例患者归入观察组。比较2组的应激反应、并发症、疼痛程度以及术后恢复情况。结果 术毕、术后1 d, 2组的皮质醇、血管紧张素Ⅱ水平及心率和平均动脉压数值均较术前1 d有所增大(P均<0.05),但观察组均小于同期对照组(P均<0.05)。观察组的并发症总发生率低于对照组(P<0.05)。术后12 h、24 h、48 h、72 h,观察组的视觉模拟评分法(VAS)评分均低于同期对照组(P均<0.05)。观察组的引流管留置时间、固体食物耐受时间、首次下床时间以及住院时间均早于或短于对照组(P均<0.05)。结论 基于ERAS理念的全程化护理管理对甲状腺癌手术患者的应激反应减轻、并发症减少、疼痛缓解以及术后恢复等均可产生积极影响。
Objective To explore the effect of implementing whole-course nursing management based on the enhanced recovery after surgery(ERAS)concept in patients with thyroid cancer undergoing surgery.Methods The clinical data of 200 patients with thyroid cancer treated at Longyan First Hospital from January 2019 to December 2021 were retrospectively analyzed.The 102 patients receiving routine nursing were included into a control group and the 98 patients receiving whole-course nursing management based on the ERAS concept were included into an observation group.The stress response,incidence of complications,degree of pain,and postoperative recovery of the two groups were compared.Results At the end of operation and 1 d after operation,the levels of cortisol and angiotensinⅡ,heart rates,and mean arterial pressure in the two groups increased compared with 1 day before operation(P<0.05),but the levels in the observation group were lower than those in the control group(P<0.05).The overall incidence of complications in the observation group was lower than that in the control group(P<0.05).At 12 h,24 h,48 h and 72 h after operation,the Visual Analogue Scale(VAS)scores in the observation group were lower than those in the control group(P<0.05)during the same period.The drainage tube retention time,solid food tolerance time,first time to ambulation,and hospital stay in the observation group were earlier or shorter than those in the control group(P<0.05).Conclusion The implementation of whole-course nursing management based on the ERAS concept can have a positive impact on reducing stress responses,minimizing complications,alleviating pain,and promoting postoperative recovery in patients with thyroid cancer undergoing surgery.
