摘要
目的 探究外周血干细胞移植术后移植物抗宿主病干眼症(GVHD-DE)患者行他克莫司治疗期间采用综合眼部护理的康复效果。方法 收集2022年3月至2023年1月在中山市人民医院血液科确诊为GVHD-DE并首次就诊于眼科的50例患者(100只眼)进行研究,依据随机数字表法分为对照组与观察组,各25例(50眼)。对照组予以常规干眼护理,观察组予以综合眼部护理。比较2组在护理前、出院时以及随访3个月的眼睛受损情况、眼部舒适度、泪膜破裂时间(BUT)和泪液分泌试验情况。结果 出院时和随访3个月时,2组的美国国立卫生研究院移植物抗宿主病(NIH GVHD)评分和眼部舒适度评分均较护理前下降(P均<0.05),且观察组的评分均较对照组低(P均<0.05);2组的BUT及泪液分泌试验对应的数值均较护理前更大(P均<0.05),且观察组均大于对照组(P均<0.05)。结论 外周血干细胞移植术后的GVHD-DE患者使用他克莫司治疗期间接受综合眼部护理,不仅有助于改善眼睛受损情况和泪膜功能,还可提升泪液分泌量以及患者的眼部舒适度。
Objective To explore the rehabilitation effect of comprehensive eye care during tacrolimus treatment in patients with graft versus host disease-associated dry eye(GVHD-DE)after the peripheral blood stem cell transplantation.Methods A total of 50 patients(100 eyes)diagnosed with GVHD-DE at the Hematology Department in their first visits at the Ophthalmology Department,Zhongshan City People's Hospital from March 2022 to January 2023 were collected.They were randomly divided into a control group and an observation group using a random number table method,with 25 cases(50 eyes)in each group.The control group received routine dry eye care,and the observation group received comprehensive eye care.The condition of eye damage,eye comfort,breakup time of tear film(BUT)and the Schirmer test before nursing,at discharge,and 3 months of follow-up were compared between the two groups.Results At discharge and 3 months of follow-up,the National Institutes of Health Graft Versus Host Disease(NIH GVHD)scores and eye comfort scores in both groups were lower than those before nursing(P<0.05),and the scores in the observation group were lower than those in the control group(P<0.05).The values of BUT and the Schirmer test in the two groups were higher than those before nursing,and the values in the observation group were higher than those in the control group(P<0.05).Conclusion The implementation of comprehensive eye care during tacrolimus treatment in GVHD-DE patients after peripheral blood stem cell transplantation is conducive to the improvement of eye damage,tear film function,lacrimal secretion,and eye comfort.
