摘要
目的基于数据挖掘分析陆付耳教授治疗2型糖尿病(type 2 diabetes mellitus,T2DM)的用药规律。方法收集2020年6月—2023年11月就诊于本院陆付耳教授门诊的T2DM患者病历资料,根据纳入、排除标准进行筛选,对数据进行标准化处理后,录入Excel 2021建立数据库,并对中药使用频次与剂量、药物功效、性味归经、方剂使用频次等进行描述性统计分析,采用R语言对药物核心网络进行提取,基于R软件中Apriori算法及Arules、ArulesViz函数对药物进行关联规则分析与可视化处理。结果年龄40~59岁就诊人数最多(407例,51.8%)。中药使用频次最高的前5位分别是甘草(778次,99.0%)、大枣(745次,94.8%)、胡芦巴(731次,93.0%)、桑白皮(695次,88.4%)、黄连(605次,77.0%)。使用频次前5位中药的常用剂量为甘草7~10 g(640次,82.3%)、大枣7~10 g(641次,86.0%)、胡芦巴21~30 g(486次,66.5%)、桑白皮11~20 g(626次,90.1%)、黄连7~10 g(259次,42.8%)。药物功效分类中频次最高的是补虚药(5046次,38.9%)。药性分布中占比最多的为温性药(3972次,30.6%)、药味分布中占比最多的为甘味药(6838次,52.7%)、归经分布中占比最多的为入肝经(3280次,25.3%)。网络分析结果显示,胡芦巴为核心药物。关联规则结果显示,胡芦巴-桑叶-桑白皮、桑叶-桑白皮、胡芦巴-肉桂-桑白皮、胡芦巴-肉桂均是置信度较高的配伍组合。方剂使用频次最高的前5位分别是扶阳解毒降糖方(388次,49.4%)、交泰丸(56次,7.1%)、补肾健脾方(29次,3.7%)、补肾通络排毒方(28次,3.6%)和健脾方(21次,2.7%)。结论陆付耳教授治疗T2DM以“益气养阴”兼顾“解毒扶阳”为整体原则,治疗方法以“扶阳固本,补肾健脾”“清热解毒,化痰除瘀”“活血通络,祛瘀生新”为核心。
Objective To analyze Professor Lu Fuer's medication rules in the treatment of type 2 diabetes mellitus(T2DM)based on data mining.Methods The medical records of T2DM patients who visited Professor Lu Fuer's outpatient clinic in our hospital from June 2020 to November 2023 were collected and screened according to inclusion and exclusion criteria.The data were standardized and entered it into Excel 2021 to establish a database.Descriptive statistical analysis was conducted on the frequency and dosage of traditional Chinese medicine use,efficacy,properties,flavor and meridian tropism,prescription usage frequency,etc.R language was used to extract the drug core network.Based on the Apriori algorithm and Arules,ArulesViz functions in R software,association rule analysis and visualization of medication were performed.Results The age group of 40~59 years old had the highest number of patients seeking medical treatment(407 cases,51.8%).The top 5 most frequently used traditional Chinese herbs are Gancao(778 times,99.0%),Dazao(745 times,94.8%),Huluba(731 times,93.0%),Sangbaipi(695 times,88.4%),and Huanglian(605 times,77.0%).The commonly used doses of the top 5 traditional Chinese herbs with the highest frequency of use are 7~10 g of Gancao(640 times,82.3%),7~10 g of Dazao(641 times,86.0%),21~30 g of Huluba(486 times,66.5%),11~20 g of Sangbaipi(626 times,90.1%),and 7~10 g of Huanglian(259 times,42.8%).The most frequent medication efficacy classification is tonifying deficiency drugs(5046 times,38.9%).The most common distribution of medicinal properties is warm drugs(3972 times,30.6%).The most common distribution of medicinal flavors is sweet flavored medicine(6838 times,52.7%).The most common meridian in the distribution is the liver meridian(3280 times,25.3%).The network analysis results show that Huluba is the core drug.The association rule results show that the combination of Huluba-Sangye-Sangbaipi,Sangye-Sangbaipi,Huluba-Rougui-Sangbaipi,Huluba-Rougui are combinations with high confidence.The top 5 formulas with the highest frequency of use are Fuyang Jiedu Jiangtang formula(388 times,49.4%),Jiaotai pill(56 times,7.1%),Bushen Jianpi formula(29 times,3.7%),Bushen Tongluo Paidu formula(28 times,3.6%)and Jianpi formula(21 times,2.7%).Conclusion Professor Lu Fuer's treatment of T2DM takes the overall principle of“benefiting qi and nourishing yin”and“detoxifying and supporting yang”.The treatment methods are centered around“strengthening yang to consolidate the foundation,tonifying kidney and strengthening spleen”,“clearing heat and removing toxicity,dissipating phlegm and eliminating blood stasis”and“promoting blood circulation for removing obstruction in collaterals,removing blood stasis for promoting tissue regeneration”.
