摘要
目的:旨在明确辨证取穴乳房穴位按摩促进生育“二孩”或“三孩”高龄产妇乳汁分泌的效果。方法:选择2022年3月—2024年3月在温州医科大学附属台州妇女儿童医院住院生育“二孩”或“三孩”的高龄产妇88例为研究对象,以随机数字表法分为对照组和观察组,每组44例。对照组产妇给予常规促进泌乳措施,观察组产妇在常规促进泌乳措施基础上给予辨证取穴乳房穴位按摩。对两组产妇的泌乳启动时间、泌乳量、催乳素水平、乳房胀痛情况、母乳喂养自信心和纯母乳喂养情况进行比较。结果:观察组的泌乳启动时间明显早于对照组(P<0.05)。产后24、48、72 h,观察组的泌乳量明显多于对照组(P<0.05)。产后72 h,观察组的催乳素水平明显高于对照组(P<0.05)。观察组中重度乳房胀痛发生率明显低于对照组(P<0.05)。干预后,观察组的母乳喂养自信心得分显著高于对照组(P<0.05)。观察组的纯母乳喂养率显著高于对照组(P<0.05)。结论:对于生育“二孩”或“三孩”的高龄产妇,辨证取穴乳房穴位按摩可以加快泌乳启动,增加泌乳量,乳房中重度胀痛发生率明显下降,有利于提高母乳喂养自信心和纯母乳喂养率。
Objective:To clarify the effect of breast acupoint massage based on dialectical acupoint selection on promoting milk secretion in elderly parturients with"second child"or"third child"childbirth.Method:A total of 88 elderly parturients who gave birth to their second or third child in Wenzhou Medical University Affiliated Taizhou Women and Children's Hospital from March 2022 to March 2024 were selected as the research subjects and divided into a control group and an observation group by the random number table method,with 44 cases in each group.The control group of parturients were given routine measures to promote lactation,and the observation group of parturients were given breast acupoint massage based on dialectical acupoint selection on the basis of routine measures to promote lactation.The lactation initiation time,milk production,prolactin levels,breast distending pain,breast-feeding confidence,and exclusive breast-feeding were compared between two groups of parturients.Result:The lactation initiation time of the observation group was significantly earlier than that of the control group(P<0.05).Twenty-four,forty-eight and seventy-two hours after delivery,the milk production of the observation group were significantly higher than those of the control group(P<0.05).Seventy-two hours after delivery,the prolactin level in the observation group was significantly higher than that in the control group(P<0.05).The incidence of moderate and severe breast distending pain in the observation group was significantly lower than that in the control group(P<0.05).After intervention,the confidence score of breast-feeding in the observation group was significantly higher than that in the control group(P<0.05).The exclusive breast-feeding rate in the observation group was significantly higher than that in the control group(P<0.05).Conclusion:For elderly parturients with"second child"or"third child"childbirth,breast acupoint massage based on dialectical acupoint selection can accelerate lactation initiation,increase milk production,significantly reduce the incidence of moderate and severe breast distending pain,which is conducive to improving breast-feeding confidence and exclusive breast-feeding rate.
