摘要
目的探讨模拟航海条件下运输摇摆对冷藏血液质量的影响。方法通过构建舰船模拟摇摆平台,将血液分为静置组和模拟运输组。将模拟运输组分为10°运输组、20°运输组和30°运输组10°运输组:海况一般;横摇角度±10°,周期8s;纵摇角度±10°,周期8s。20°运输组:海况中等;横摇角度±20°周期8s;纵摇角度±10°,周期8s。30°运输组:海况恶劣;横摇角度±30°周期8s;纵摇角度±10°,周期8s。模拟不同海况条件,每天持续12h摇摆后静置12h,循环3次进行。对血袋外观、游离血红蛋白(FHb)与溶血率、pH值、血细胞分析[白细胞(WBC)、红细胞(RBC)、平均红细胞体积(MCV)、红细胞比容(Hct)、血小板(PLT)、RBC渗透脆性和血液黏度进行检测。结果随着保存时间的延长,所有组的血液质量均有所下降,运输组下降更为显著。具体表现为FHb和溶血率随海况等级恶劣而增加,其中30°运输组FHb(714.47mg/L±71.88mg/L)较静置组(353.15mg/L±34.17mg/L)升高了近2倍(P<0.05);在摇摆程度最大的30°运输组PLT指标下降最缓慢(下降44%),而静置组下降最明显(下降约72%)(P<0.05);RBC渗透脆性显著增大(P<0.05),血液黏度升高在第24天的时候尤为明显(P<0.05);各组间WBC、RBC、MCV、Hct与pH无统计学意义(P>0.05)。结论航海条件下运输会对冷藏血液制品产生不利影响,且影响程度随海况恶劣而加剧。该研究对于海上救援和临床输血安全具有重要意义。
Objective To investigate the effect of transportation swing on the quality of refrigerated blood under simulated navigation conditions.Methods The ship simulated swing platform was constructed,and the blood was divided into static group and simulated transportation group.The simulated transportation group was divided into 1oo transportation group(general navigation condition:lateral swing angle±10°with period 8-second;longitudinal swing angle±10°with period 8-second),20°transportation group(medium navigation condition:lateral swing angle±20°with period 8-second.longitudinal swing angle±10°with period 8-second)and 30°transportation group(poor navigation condition:lateral swing angle±30°with period 8-second;longitudinal swing angle±10°with period 8-second).Different navigation conditions were simulated,the swing condition for 12 hours followed by static condition for 12 hours in daily were performed and cycled 3 frequencies.The appearance of storage blood bag,free hemoglobin(FHb)and hemolysis rate,pH value,blood cell analysis[white blood cell(WBC),red blood cell(RBC),mean corpuscular volume(MCV),hematocrit(Hct),platelet(PLT)l,RBC osmotic fragility and blood viscosity were detected.Results The blood quality of all groups decreased with the prolongation of storage time,and the decrease in simulated transportation group was more significant.The FHb and hemolysis rate increased with the severity of navigation conditions.Among them,the FHb in 30°transportation group(714.47 mg/L±71.88 mg/L)was nearly twice as high as that in static group(353.15 mg/L±34.17 mg/L)(P<0.05).The PLT index in 300 transportation group(the greatest swing degree)decreased the slowest(decreased by 44%),while that in static group decreased the most significantly(decreased by approximately 72%)(P<0.05).The osmotic fragility of RBC increased significantly(P<0.05),and blood viscosity increased significantly on 24th day(P<0.05).There was no significant difference in WBC,RBC,MCV,Hct and pH in each group(P>O.05).Conclusion It is demonstrated that navigation transportation may have adverse effect on refrigerated blood products,degree of influence intensifies with the severity of sea conditions,so is of great significance for maritime rescue and clinical blood transfusion safety.
