摘要
目的探讨左心室心肌整体纵向应变(GLS)及做功参数在左心室射血分数(LVEF)正常冠心病患者左心室收缩功能评估中的应用价值。方法选择我院收治的59例LVEF正常冠心病患者为观察组,另以同期健康体检的71名志愿者为对照组。应用超声心动图及心功能成像分析软件获得左心室常规收缩功能参数、GLS及做功参数。用受试者工作特征(ROC)曲线计算曲线下面积(AUC),以分析各指标对LVEF正常冠心病患者左心室收缩功能的评估效能。结果观察组的左心室收缩末期容积(LVESV)、左心室舒张末期容积(LVEDV)大于对照组,左心室后壁厚度(LVPWT)小于对照组(P<0.05)。观察组的GLS、整体做功指数(GWI)、整体有效做功(GCW)、整体做功效率(GWE)低于对照组,整体无效做功(GWW)高于对照组(P<0.05)。GLS、GWI、GCW、GWE与LVESV、LVEDV呈负相关,GWW与LVESV、LVEDV呈正相关,与LVPWT呈负相关(P<0.05)。GLS、GWI、GCW、GWW、GWE评估LVEF正常冠心病患者左心室收缩功能的AUC分别为0.831、0.788、0.844、0.900、0.917。结论GLS及做功参数可有效评估LVEF正常冠心病患者的左心室收缩功能。
Objective To explore the application value of global longitudinal strain(GLS)and work parameters of left ventricular myocardium in the evaluation of left ventricular systolic function in patients with coronary heart disease with normal left ventricular ejection fraction(LVEF).Methods A total of 59 patients with coronary heart disease with normal LVEF admitted in our hospital were selected as observation group,and 71 volunteers who underwent physical examination during the same period were selected as control group.Conventional left ventricular systolic function parameters,GLS and work parameters were obtained by echocardiography and cardiac function imaging analysis software.The area under the curve(AUC)was calculated by the receiver operating characteristic(ROC)curve to analyze the evaluation efficacy of each index on left ventricular systolic function in patients with coronary heart disease with normal LVEF.Results The left ventricular end-systolic volume(LVESV)and left ventricular end-diastolic volume(LVEDV)in the observation group were greater than those in the control group,and the left ventricular posterior wall thickness(LVPWT)was less than that in the control group(P<0.05).The GLS,global work index(GWI),global constructive work(GCW)and global work efficiency(GWE)of the observation group were lower than those of the control group,and the global waste work(GWW)was higher than that of the control group(P<0.05).GLS,GWI,GCW and GWE were negatively correlated with LVESV and LVEDV,and GWW was positively correlated with LVESV and LVEDV,and negatively correlated with LVPWT(P<0.05).The AUC of GLS,GWI,GCW,GWW and GWE in evaluating left ventricular systolic function in patients with normal LVEF were 0.831,0.788,0.844,0.900 and 0.917,respectively.Conclusion GLS and work parameters can effectively evaluate left ventricular systolic function in patients with coronary heart disease with normal LVEF.
