摘要
目的观察电针深刺八髎穴配合敛肠汤灌肠治疗溃疡性结肠炎(ulcerative colitis,UC)的效果及对肠道黏膜屏障功能、白细胞介素(interleukin,IL)-23/IL-17炎性轴的影响。方法选取2021年1月—2023年12月该院收治的80例UC患者,按随机数字表法分为对照组和观察组,各40例,对照组给予常规西医治疗,观察组在对照组基础上给予敛肠汤灌肠结合电针深刺八髎穴治疗,比较两组临床疗效、中医证候积分、肠道黏膜屏障功能[D-乳酸(D-LA)、内毒素(BT)、二胺氧化酶(DAO)]、血清炎症因子(IL-23、IL-17)、外周血Treg/Th17细胞和不良反应发生情况。结果观察组治疗有效率高于对照组[92.50%(37/40)vs 75.00%(30/40)](P<0.05)。治疗后两组中医证候积分及血清D-LA、BT、DAO、IL-23、IL-17水平较治疗前均降低,且观察组低于对照组(P<0.05)。治疗后两组Th17/Treg较治疗前升高,且观察组高于对照组(P<0.05)。两组不良反应发生率比较差异无显著统计学意义(P>0.05)。结论敛肠汤灌肠结合电针深刺八髎穴治疗UC的疗效显著,可缓解患者症状,改善肠道黏膜屏障功能,其作用机制可能与调节IL-23/IL-17炎性轴和改善Treg/Th17细胞免疫失衡有关。
Objective To observe the effect of deep electroacupuncture at Baliao acupoints combined with enema with Lianchang Decoction(敛肠汤)in the treatment of ulcerative colitis(UC),and the impact on intestinal mucosal barrier function and interleukin(IL)-23/IL-17 axis.Methods Eighty UC patients admitted to the hospital from January 2021 to December 2023 were selected and randomly divided into control group and observation group with 40 cases in each.The patients in the control group were given conventional western medicine treatment,and those in the observation group were treated with enema with Lianchang Decoction combined with deep electroacupuncture at Baliao acupoint on this basis.The clinical effects,traditional Chinese medicine syndrome scores,intestinal mucosal barrier function[D-lactic acid(D-LA),endotoxin(BT)and diamine oxidase(DAO)],serum inflammatory factors(IL-23 and IL-17),peripheral blood Treg/Th17 cells,and the incidence of adverse reactions were compared between the two groups.Results The total effective rate of observation group was higher than that of control group[92.50%(37/40)vs.75.00%(30/40)](P<0.05).After treatment,TCM syndrome scores and serum levels of D-LA,BT,DAO,IL-23 and IL-17 in both groups were lower than those before treatment,and the levels of the observation group were lower than those of the control group(P<0.05).After treatment,Th17/Treg in both groups was higher than that before treatment,and the ration of the observation group was higher than that of the control group(P<0.05).There was no difference in adverse reactions between the two groups(P>0.05).Conclusion Enema with Lianchang Decoction combined with deep electroacupuncture at Baliao acupoints is significantly effective in the treatment of UC,which can alleviate symptoms and improve intestinal mucosal barrier function.The mechanism may be related to regulating IL-23/IL-17 axis and improving Treg/Th17 imbalance.
