摘要
目的探究化痰活血消瘿散联合针灸对良性甲状腺结节(benign thyroid nodules,BTN)患者的效果及对甲状腺功能、结节内血流信号的影响。方法选取该院2022年6月—2023年8月收治的气郁痰阻证BTN患者,随机数字表法分为两组,每组40例,对照组采用针灸治疗,观察组予以针灸治疗联合化痰活血消瘿散外敷。记录两组患者治疗3个月后的临床疗效变化;记录两组患者治疗前及治疗3个月后的结节内血流信号分级情况、中医证候积分、结节情况(个数、峡部厚度、直径、体积)、甲状腺功能[游离三碘甲状腺原氨酸(free triiodothyronine,FT3)、游离甲状腺素(free thyroxine,FT4)、促甲状腺激素(thyroid stimulating hormone,TSH)]。结果治疗3个月后,观察组疗效有效率显著高于对照组(P<0.05);治疗3个月后,观察组结节内血流信号等级显著低于对照组(P<0.05),两组患者中医证候积分、结节个数、峡部厚度、结节直径、结节体积、TSH水平显著降低,且观察组低于对照组(P<0.05),FT3和FT4水平差异无统计学意义(P>0.05)。结论联合治疗能帮助改善BTN患者的临床症状与结节情况,提高结节内血流信号分级情况,有助于恢复患者甲状腺功能和肝肾功能,值得临床推广使用。
Objective To explore the effect of Huatan Huoxue Xiaoying Powder(化痰活血消瘿散)combined with acupuncture and moxibustion on patients with benign thyroid nodules(BTN)and its influence on thyroid function and blood flow signals in nodules.Methods The patients with BTN of Qi stagnation and phlegm obstruction syndrome in the hospital were selected from June 2022 to August 2023 and were classified into two groups by means of the random number table method,with 40 cases in each group.The control group was treated with acupuncture and moxibustion,while the observation group was given acupuncture and moxibustion combined with external application of Huatan Huoxue Xiaoying Powder.The clinical efficacy after 3 months of treatment and blood flow signal grading in nodules,TCM syndromes scores,nodules(count,isthmus thickness,diameter,volume)and thyroid function[free triiodothyronine(FT3),free thyroxine(FT4),thyroid stimulating hormone(TSH)]before treatment and after 3 months of treatment were recorded in the two groups.Results After 3 months of treatment,the effective rate of efficacy in the observation group was obviously higher than that in the control group(P<0.05).The blood flow signal grading in nodules in the observation group after 3 months of treatment was significantly lower compared to that in the control group(P<0.05).The TCM syndromes scores,nodule count,isthmus thickness,nodule diameter,nodule volume and TSH level were declined obviously in both groups,and the above indicators were lower in the observation group(P<0.05),but there were no statistical differences in the levels of FT3 or FT4 between both groups(P>0.05).Conclusion Combination therapy can help to improve the clinical symptoms and nodules,improve the blood flow signal grading in nodules,and help to restore the thyroid function and liver and kidney function in patients with BTN.
