摘要
基于胃癌前状态“食毒”学术思想提出治胃八法并分析其方药配伍特色。认为“食毒”累积于胃、损伤胃腑是造成胃癌前状态的重要因素,治疗上以消食强胃法、降气理脾法为核心,运用“炒麦芽、炒稻芽”“鸡内金、白术”等药对恢复胃腐熟水谷的生理功能,“枳实、白术”“沉香、槟榔”等配伍恢复胃主通降的生理特性。对于调和肝胃法、温中暖胃法、除湿化浊法、滋养胃阴法、解毒安胃法的选择则根据具体“食毒”类型和癌变风险的不同采取相应治法,并以饮食调摄法为辅。治胃八法着眼于胃,立足于岭南地区,体现了对胃与脾、肝之间关系的重视,关注了轻重、缓急的病势动态变化,按照疾病在气分、血分或轻中重程度的区别分层分级配伍用药,可以为中医药治疗胃癌前状态提供新思路。
This paper proposed eight methods of treating gastric precancerous conditions and analyzed the characteristics of their prescription compatibility based on the academic thought of“diet toxin”.It is believed that the accumulation of“diet toxin”in the stomach as well as the damage to the stomach is an important factor contributing to the gastric precancerous conditions.The treatments are centered on promoting digestion and strengthening stomach,lowering Qi and regulating spleen,restoring the physiological function of the stomach to putrefy grains by applying drug pairs such as“fried Maiya(Malt)and fried Daoya(Rice Sprout)”,“Jineijin(Chicken Gizzard Endothelium)and Baizhu(Atractylodis Macrocephalae Rhizoma)”and so on,restoring the physiological characteristic of the stomach to pass and descend by applying drug pairs such as“Zhishi(Immature Bitter Orange)and Baizhu(Atractylodis Macrocephalae Rhizoma)”,“Chenxiang(Aquilaria Agallocha)and Binglang(Areca Catechu)”,etc.The methods of harmonizing liver and stomach,warming stomach,removing dampness and turbidity,nourishing stomach Yin,detoxifying and calming stomach are adopted according to different types of“diet toxin”and cancer risk,supplemented by dietary adjustment.Focusing on the stomach and the region of southern China,the eight methods of treating gastric precancerous conditions reflect the importance of the relationship between stomach-spleen and liver.They pay attention to the dynamic changes in the severity and urgency of the disease,guide medication and compatibility according to the differentiation of Qi,blood or the degree of severity.This paper could provide new insights for the treatment of gastric precancerous conditions with traditional Chinese medicine.
