张艳基于“心-脑-肾轴”理论辨治慢性心衰利尿剂抵抗的临床经验

ZHANG Yan′s Clinical Experience in Treating Diuretic Resistance of Chronic Heart Failure Based on Theory of“Heart-Brain-Kidney Axis”

摘要 慢性心力衰竭(chronic heart failure,CHF)是多种心血管疾病的终末阶段,主要以药物治疗为主,介入、手术指征较少。因CHF患者常出现液体潴留现象,故利尿剂在CHF的治疗中扮演关键作用,但研究表明近三分之一的患者在治疗过程中发生利尿剂抵抗,导致CHF预后差、患者病死率高。该研究总结了张艳教授四十余年临床诊治慢性心衰的经验,从“心-脑-肾轴”理论辨治慢性心衰利尿剂抵抗。从血脉互滋、精神互利、督脉相通三方面加以论述,应用补心温肾安神法,提出“心-脑-肾轴同调”为治疗大法,在临床应用中取得了较为满意的治疗效果,为中医药治疗慢性心衰利尿剂抵抗提供了新的思路和方向。 Chronic heart failure(CHF)is the terminal stage of a variety of cardiovascular diseases,mainly with drug therapy,fewer indications for intervention and surgery.The patients with CHF often have fluid retention,and diuretics play a key role in the treatment of CHF.However,nearly one third of the patients have diuretic resistance during the treatment process,with poor prognosis and high mortality.This study summarized Professor ZHANG Yan′s experience in clinical diagnosis and treatment of CHF for more than 40 years,and differentiated and treated diuretic resistance of CHF from the theory of“heart-brain-kidney axis”.This article discussed the three aspects of mutual nourishing of blood vessels,mutual benefit of spirit,and communication of Du meridian,and applied the method of tonifying heart,warming kidney and calming the nerves.It put forward the simultaneously adjusting“heart-brain-kidney axis”method as the major treatment method,which had achieved satisfactory therapeutic effect in clinical application,providing a new idea and direction for traditional Chinese medicine to treat diuretic resistance in chronic heart failure.
