摘要
目的探讨体重调整腰围指数(weight-adjusted waist index,WWI)与高血压合并高尿酸血症(hyperten-sion plus hyperuricemia,HTN-HUA)之间的关联性,并分析其对高血压(hypertension,HTN)及高尿酸血症(hy-peruricemia,HUA)单独状态的预测价值。方法本研究基于中国健康与养老追踪调查第三轮数据,纳入9801例≥45岁的中老年受试者。采用Logistic回归模型分析WWI与HTN-HUA、单纯HTN及单纯HUA的关联关系;采用限制性立方样条(restricted cubic spline,RCS)分析WWI与HTN-HUA的非线性关系,并绘制受试者工作特征(receiver operating characteristic,ROC)曲线评估WWI的预测效能。结果纳入9801例参与者中,HTN-HUA患者756例(8%),单纯HTN 4381例(45%),单纯HUA 1236例(13%)。多因素调整后,WWI与HTN-HUA、单纯HTN及单纯HUA均呈显著正相关。最高四分位组的HTN-HUA、HTN及HUA的发生风险分别是WWI最低四分位组的3.04倍(95%CI:2.35~3.94)、1.53倍(95%CI:1.34~1.74)和1.93倍(95%CI:1.42~2.61)。RCS分析提示WWI与HTN-HUA之间存在非线性关联。分层分析显示,该关联在45~59岁人群及无糖尿病者中更为显著。结论WWI与高血压合并高尿酸血症呈显著且非线性相关。WWI或可用于HTN-HUA的早期识别。
Objective To explore the correlation between weight-adjusted waist index(WWI)and hypertension plus hyperuricemia(HTN-HUA),and to analyze its predictive value for hypertension(HTN)and hyperuricemia(HUA).Methods Based on the data of the third round of the China Health and Retirement Longitudinal Study,this study in-cluded 9801 middle-aged and elderly subjects over forty-five years old.Logistic regression model was used to ana-lyze the correlation between WWI and HTN-HUA,simple HTN and simple HUA.The restricted cubic spline(RCS)was used to analyze the nonlinear relationship between WWI and HTN-HUA,and the receiver operating characteris-tic(ROC)curve was drawn to evaluate the predictive efficacy of WWI.Receiver operating characteristic(ROC)curves were utilized to assess the predictive ability of WWI.Results Among the 9801 participants,756(8%)had HTN-HUA,4381(45%)had simple HTN,and 1236(13%)had simple HUA.After multivariate adjustment,WWI was sig-nificantly positively correlated with HTN-HUA,simple HTN and simple HUA.The risks of HTN-HUA,HTN,and HUA in the highest quartile group were 3.04 times(95%CI:2.35-3.94),1.53 times(95%CI:1.34-1.74),and 1.93 times(95%CI:1.42-2.61)those of the lowest quartile group of WWI,respectively.RCS analysis suggests that there is a nonlinear correlation between WWI and HTN-HUA.Stratified analysis showed that the association was more signifi-cant in people aged 45-59 years and those without diabetes.Conclusion WWI was significantly and non-linearly cor-related with hypertension complicated with hyperuricemia.WWI may be used for early identification of HTN-HUA.
