摘要
卒中后肺炎是脑卒中常见并发症之一,严重影响患者的康复进程。西医治疗主要包括抗生素抗感染和镇咳、化痰、补液、吸氧等对症措施。这些措施能有效缓解症状,但缺点是易导致耐药性和病情反复发作。相比之下,中医思维强调整体观念,治疗该疾病有一定优势。“一气周流”为黄元御学术思想的核心,其秉承中医学整体观,认为中焦脾胃为人体枢轴,中焦斡旋带动全身气机升降。根据这一理论,卒中后肺炎的核心病机为中土斡旋失司、气机紊乱,进而引发肝脾不和、木火刑金、肺气宣降失常、病理产物内生,从而引发该疾病。治疗以调理中焦脾胃升降为主,辅以宣肺理气、疏肝利胆之法,以恢复气机的升降平衡,促进气血的周流不息。从“一气周流”理论分析卒中后肺炎的病因病机及治疗思路,体现了中医整体观念和调畅气机在疾病治疗中的关键作用,以期为临床实践提供参考。
Post-stroke pneumonia is one of the common complications of stroke,which seriously affects the recovery process of patients in the post-stroke period.Western medical treatment primarily encompasses antibiotics for anti-infection purposes,along with symptomatic measures such as cough suppression,expectoration relief,fluid replenishment,and oxygen therapy.These measures can effectively alleviate symptoms,but they have the drawback of easily leading to drug resistance and recurrent episodes of the illness.In contrast,Chinese medicine emphasizes the holistic concept and has certain advantages in treating the disease.The core of Huang Yuanyu s academic thinking is“The Uninterrupted Circulation of Qi”,which adheres to the holistic view of Chinese medicine and believes that the middle warmer is the pivot of the human body and that the middle warmer mediates the lifting and lowering of the whole body s Qi.According to this theory,the core pathomechanism of post-stroke pneumonia is a failure of the mediation of the middle warmer and a disturbance of Qi,leading to disharmony between the liver and the spleen,punishment of metal by wood and fire,malfunction of lung Qi in declaring and descending,and internalization of pathological products,which triggers the disease.The treatment mainly focuses on regulating the elevation of the spleen and stomach in the middle jiao,supplemented by methods of promoting the lungs,regulating Qi,and dredging the liver and gallbladder,to restore the balance of the elevation of Qi and promote the uninterrupted flow of Qi and blood.Analyzing the etiology and treatment of post-stroke pneumonia from the theory of“The Uninterrupted Circulation of Qi”reflects the key role of the overall concept of Chinese medicine and the regulation of Qi in the treatment of the disease,to provide a useful reference for clinical practice.
