摘要
目的探究腰椎间盘突出症患者住院费用结构及影响因素。方法抽取安徽省某三甲医院2018-2022年的腰椎间盘突出症患者临床数据进行回顾性分析,共纳入400例;分析例均住院总费用及支出项目变化,计算各项费用支出项目结构变动值;使用单因素分析和多重线性回归分析住院费用的影响因素。结果该院2018-2022年腰椎间盘突出症患者住院费用呈现逐年升高趋势,支出项目中构成比自低到高为:检验费<床位费<材料费<治疗费<药费,治疗费和药费为主要费用支出项目,其构成比分别为>50.00%和>20.00%;多重线性回归分析结果显示,年龄≥60岁、外埠、自费、住院天数≥15 d与腰椎间盘突出症患者住院费用呈正相关(β=0.157、0.148、0.166、0.174,P=0.023、0.018、0.028、0.032)。结论医院2018-2022年腰椎间盘突出症患者的住院费用较高,且以治疗费和药费为主要项目,其影响因素包括年龄、来源区域、支付方式以及住院天数,可针对不同年龄、来源区域、支付方式以及住院天数的腰椎间盘突出症患者制定合理的医药费政策。
Objective To explore the structure and influencing factors of hospitalization expenses of patients with lumbar disc herniation in a tertiary hospital.Methods The clinical data of patients with lumbar disc herniation in a ClassⅲGrade A hospital in Anhui Province from 2018 to 2022 were selected for retrospective analysis,and a total of 400 cases were included.The changes of total hospitalization expenses and expenditure items per case were analyzed,and the structural change value of each expenditure item was calculated.Single factor/multiple linear regre-ssion analysis was used to analyze the influencing factors of hospitalization expenses.Results From 2018 to 2022,the hospitalization expenses of patients with lumbar disc herniation showed an increasing trend year by year.The constituent ratio of the expenditure items from low to high was:examination cost<bed cost<material cost<treatment cost<drug cost.Multiple linear regression analysis showed that the hospitalization expenses of patients with lumbar disc herniation were positively correlated with age≥60 years old,non-local area,self-paid,and hospitalization days≥15 days(β=0.157,0.148,0.166,0.174;P=0.023,0.018,0.028,0.032).Conclusion The hospitalization cost of patients with lumbar disc herniation in the hospital from 2018 to 2022 is relatively high,and the treatment expenses and drug expenses were the main items.The influencing factors included age,source region,payment method and length of hospital stay.Reasonable medical expenses policies can be formulated for lumbar disc herniation patients with different ages,source regions,payment methods and length of hospital stay.
