摘要
目的:探讨综合医院早产儿住院费用负担和费用构成,为特殊人群医保支付提供依据。方法:总结2015—2021年于武汉大学中南医院NICU住院的早产儿临床资料,收集出院早产儿住院费用,根据支付项,费用分解为药费、床位费、治疗费、检验费、材料费和影像学费用等。分别按出生胎龄(GA)和出生体质量(BW)分组对数据进行统计分析。结果:本研究胎龄中位数34周,出生体质量中位数2090 g,住院费用中位数为20494(13120,33323)元,住院时间11(7,17)d。早产儿住院费用7年间增长81%;存活超早产儿及GA28周早产儿的住院费用显著高于其他胎龄组,分别为82457元和67256元;超低和极低出生体质量儿住院费用为67039元和50580元。住院费用以治疗费占比最高,达49.6%。结论:近7年NICU住院早产儿医疗费用逐渐增加,随早产儿胎龄和出生体质量降低而增高,住院费用以治疗费用增加为主;早产儿按胎龄分组较体质量分组更准确反映早产儿疾病实际需要支付的住院费用。
Objective:To explore the burden and composition of hospitalization expenses of preterm infants in general hospitals,and to provide evidence for medical insurance payment of special residents.Methods:Clinical data and hospitalization expenses of preterm infants hospitalized in the NICU of Zhongnan Hospital of Wuhan University from 2015 to 2021 were collected and summarized.According to payment items,the expenses were decomposed into drug expenses,bed expenses,treatment expenses,examination fees,materials fees and imaging fees,etc.The data were statistically analyzed by birth gestational age(GA)and body weight(BW).Results:The median gestational age of preterm infants in this study was 34 weeks,the median birth weight was 2090 g,the median hospitalization cost was 20494(13120,33323)yuan,and the length of hospitalization was 11(7,17)days.The hospital expenses of preterm infants increased by 80%in 7 years.The hospitalization expenses of surviving extremely preterm infants and GA 28‑week preterm infants were significantly higher than those of other GA groups.The hospital expenses for extremely low and very low birth weight infants were 67039 yuan and 50580 yuan.Conclusion:In the recent 7 years,the medical expenses of hospitalized preterm infants in NICU increased gradually,and increased with the decrease of gestational age and birth weight,with treatment costs being the main contributor to the increase.Grouping preterm infants by gestational age is more accurate than by birth weight in reflecting the actual hospitalization costs required for their diseases.
