摘要
目的探究糖皮质激素联合低分子肝素治疗孕早期绒毛膜下血肿(SCH)的效果。方法择取2024年3月至2025年2月之间,在我院因先兆流产住院的200例患者,进行回顾性分析。根据是否合并SCH及血肿/孕囊面积比例法,将所有的患者划分为3组,即无SCH组、轻度SCH组(血肿/孕囊面积<1/2)和重度SCH组(血肿/孕囊面积>1/2)。就三组孕妇的流产率、流产时孕周、妊娠活产率,妊娠期并发症、PLT、D-D、PT、TT、APTT、FIB、SCH吸收所用时间展开比较。结果三组的流产率、流产时孕周、妊娠活产率、妊娠期并发症、PLT、D-D、PT、TT、APTT、FIB比较,结果P>0.05;重度SCH组的SCH吸收所用时间比轻度SCH组更长(P<0.05)。比较三组的不良反应发生率,其结果的一致性比较好(P>0.05)。结论糖皮质激素联合低分子肝素在治疗孕早期SCH中具有显著的效果,能有效地缩小血肿的面积,减少流产的风险,而且不良反应的风险比较低,建议临床推广。
Objective To investigate the effect of glucocorticoid combined with low molecular heparin in the treatment of subchorionic hematoma(SCH)in early pregnancy.Methods A total of 200 patients who were hospitalized for preeclampsia in our hospital between March 2024 and February 2025 were selected for retrospective analysis.All patients were categorized into three groups according to the combination of SCH and hematoma/gestational sac area ratio method,namely,no SCH group,mild SCH group(hematoma/gestational sac area<1/2),and severe SCH group(hematoma/gestational sac area>1/2).Miscarriage rate,gestational week at the time of miscarriage,live birth rate,complications during pregnancy,PLT,D-D,PT,TT,APTT,FIB,and time taken for SCH absorption were compared among the three groups.Results Comparison of miscarriage rate,gestational week at the time of miscarriage,gestational live birth rate,gestational complications,PLT,D-D,PT,TT,APTT,FIB among the three groups were no significant difference(P>0.05);the time taken for SCH absorption was longer in the severe SCH group than in the mild SCH group(P<0.05).The incidence of adverse reactions was compared among the three groups,and the consistency of the results was relatively good(P>0.05).Conclusion Glucocorticoid combined with low molecular heparin has a significant effect in the treatment of SCH in early pregnancy,which can effectively reduce the size of hematoma and the risk of miscarriage,and the risk of adverse reactions is relatively low,so it is recommended to be promoted clinically.
