摘要
目的探讨孕激素诱导封闭因子(PIBF)在绒毛膜下血肿(SCH)患者体内的表达及与Th1/Th2细胞因子的相关性。方法选取2021年1月至12月新余市人民医院80例先兆流产孕妇为研究对象,其中合并SCH者40例纳入A组,单纯阴道出血未合并SCH者40例纳入B组,另外选择正常妊娠孕妇40名纳入C组。观察先兆流产孕妇的妊娠结局,比较三组的血清PIBF、激素[雌二醇(E_(2))、孕酮(P)、人绒毛膜促性腺激素(β-HCG)]水平、Th1型细胞因子[肿瘤坏死因子-α(TNF-α)]和Th2型细胞因子[白介素6(IL-6)]的表达,分析不同妊娠结局先兆流产合并SCH患者血清PIBF与E_(2)、P、β-HCG、Th1/Th2水平的关系。结果A组患者血清PIBF及E_(2)、P、β-HCG水平低于B组与C组,B组血清PIBF及E_(2)、P、β-HCG水平低于C组,差异有统计学意义(P<0.05);A组患者血清TNF-α水平高于B组与C组,IL-6水平低于B组与C组,差异有统计学意义(P<0.05);B组患者血清TNF-α水平高于C组,IL-6水平低于C组,差异有统计学意义(P<0.05)。A组保胎成功率为57.50%,低于B组的80.00%,差异有统计学意义(P<0.05)。合并SCH先兆流产孕妇安胎成功者血清PIBF及E_(2)、P、β-HCG水平以及IL-6水平高于安胎失败者,TNF-α水平低于安胎失败者,差异有统计学意义(P<0.05)。Pearson相关性分析结果显示合并SCH先兆流产孕妇血清PIBF水平与激素E_(2)、P、β-HCG水平、Th2细胞因子IL-6呈正相关(P<0.05),与Th1细胞因子TNF-α水平呈负相关(P<0.05)。结论合并SCH的先兆流产孕妇PIBF呈现低表达,且PIBF表达水平下调可影响Th1/Th2平衡状态向Th1细胞因子方向偏移,增加流产风险。
Objective To explore the expression of progesterone-induced blocking factor(PIBF)and its correlation with Th1/Th2 cytokines in patients with subchorionic hematoma(SCH).Methods Eighty pregnant women with threatened abortion from Xinyu People's Hospital from January to December 2021 were selected as the study objects,among which 40 patients with SCH were included in group A,40 patients with simple vaginal bleeding without SCH were included in group B,and 40 pregnant women with normal pregnancy were included in group C.The pregnancy outcomes of pregnant women with threatened abortion were observed.The differences in levels of serum PIBF and hormones(estradiol[E_(2)],progesterone[P],human chorionic gonadotropin[β-HCG]),and expressions of Th1 cytokines(tumor necrosis factor-α[TNF-α])and Th2 cytokines(interleukin 6[IL-6])were compared among the three groups.The relationship between serum PIBF and E_(2),P,β-HCG,Th1/Th2 was analyzed.Results The serum PIBF and E_(2),P andβ-HCG levels of group A were lower than those of group B and C,and the serum PIBF and E_(2),P andβ-HCG levels of group B were lower than those of group C,and the differences were statistically significant(P<0.05).The serum TNF-αlevel of group A was higher than that of group B and C,and IL-6 level was lower than that of group B and C,and the differences were statistically significant(P<0.05).The serum TNF-αlevel of group B was higher than that of group C,and the serum IL-6 level was lower than that of group C,with statistical significances(P<0.05).The success rate of group A was 57.50%lower than that of group B(80.00%),the difference was statistically significant(P<0.05).The levels of PIBF,E_(2),P,β-HCG and IL-6 in the pregnant women with SCH threatened abortion were higher than those in the pregnant women with safe abortion,and the levels of TNF-αwere lower than those in the pregnant women with safe abortion,the differences were statistically significant(P<0.05).Pearson correlation analysis showed that the serum PIBF level of pregnant women with SCH threatened abortion was positively correlated with the levels of hormone E_(2),P,β-HCG and Th2 cytokine IL-6(P<0.05),and negatively correlated with the level of Th1 cytokine TNF-α(P<0.05).Conclusion The expression of PIBF is down-regulated in threatened abortion women with SCH,which will affect the shift of Th1/Th2 balance to Th1 cytokines and increase abortion risk.
