摘要
儿童颅咽管瘤导致的视神经病变临床上属难治之症,中医治疗在此有独特优势。颅咽管瘤作为儿童常见的先天性颅内肿瘤,常导致视神经病变,表现为视力下降、视野缺损等症状,且术后易出现内分泌紊乱等并发症。韦企平结合现代医学检查手段拓展四诊,结合患儿体质进行论治,并对患儿进行精准辨证。其认为该病病机复杂,与胎禀不足、肾精亏虚、脾失健运、气机郁滞等因素密切相关;治疗上,明分期、重辨证、调气机、补五脏、散瘀结,和阴阳;采用“青盲一号方”化裁联合“韦氏三联九针”法,注重调节患儿整体状态,改善视功能。通过临床案例展示了其治疗方法的有效性,为治疗此类难治性眼病提供了宝贵经验。
Optic neuropathy caused by craniopharyngioma in children is clinically refractory,and Traditional Chinese Medicine(TCM)demonstrates unique advantages in its treatment.Craniopharyngioma,a common congenital intracranial tumor in children,often leads to optic neuropathy manifested as visual impairment,visual field defects,and postoperative complications such as endocrine disorders.WEI Qiping extends the four diagnostic methods by integrating modern medical examinations,tailors treatment based on the child´s constitution,and conducts precise syndrome differentiation.He attributes the complex pathogenesis to factors including congenital deficiency,kidney essence deficiency,spleen dysfunction,and Qi stagnation.The therapeutic principles involve staging differentiation,emphasizing pattern identification,regulating Qi movement,tonifying the five zang organs,resolving stasis and nodules,and harmonizing Yin-Yang.The modified"Qingmang Formula I"combined with"WEI´s Triple-Nine Acupuncture"focuses on regulating the child´s holistic state and improving visual function.Clinical cases demonstrate the efficacy of this approach,providing valuable experience for treating such refractory ophthalmic conditions.
