国产脉冲电场消融系统治疗阵发性心房颤动患者的安全性及效果分析

Safety and efficacy analysis of the domestic pulsed field ablation system for the treatment of paroxysmal atrial fibrillation

摘要 目的:旨在评估一种新型国产脉冲电场消融(pulsed field ablation,PFA)系统在治疗阵发性心房颤动患者中的安全性及效果。方法:入选2022年7月至2023年7月于郑州大学附属河南省胸科医院心内科接受首次导管消融治疗的阵发性心房颤动患者22例,随机数字表法随机分为两组,脉冲消融组(8例)与射频消融组(14例)。全部患者在消融前后分别进行左心房三维电解剖和心房基质标测,术后随访1年。结果:两组患者全部均达到肺静脉隔离的手术终点。围手术期及术后1年随访期间,未发生手术相关不良事件。与射频消融组相比,脉冲消融组患者治疗时间明显减少[(31.6±12.2)vs.(92.4±35.0)min,P<0.001]。结论:PFA技术在治疗心房颤动上是安全的。与射频消融治疗阵发性心房颤动相比,新型PFA系统治疗阵发性心房颤动患者的消融时间缩短,但该结果仍需大样本及长期随访结果加以验证。 Objective:To evaluate the safety and efficacy of a new domestic pulsed field ablation(PFA)system in the treatment of paroxysmal atrial fibrillation(AF).Methods:Twenty-two patients with paroxysmal AF who underwent their first catheter ablation treatment in the Department of Cardiovascular Medicine of Henan Provincial Chest Hospital affiliated with Zhengzhou University from July 2022 to July 2023 were enrolled,and were randomly divided into two groups,the PFA group(8 patients)and the radiofrequency ablation group(14 patients).All patients underwent three-dimensional electroanatomic and atrial substrate mapping of the left atrium before and after ablation,with a follow-up of one year postoperatively.Results:All patients in two groups achieved the surgical endpoint of pulmonary vein isolation.No procedure-related adverse events occurred during the perioperative period and 1-year postoperative follow-up.Compared with the radiofrequency ablation group,patients in the PFA group had a significantly reduced treatment time[(31.6±12.2)vs.(92.4±35.0)min,P<0.001].Conclusions:The PFA technique is safe in the treatment of AF.Compared with radiofrequency ablation for the treatment of paroxysmal AF,the ablation time of patients with paroxysmal AF treated with the new PFA system was shortened,but this result still needs to be validated with a large sample and long-term follow-up results.
