摘要
目的 分析沙库巴曲缬沙坦结合达格列净在老年糖尿病合并冠心病伴肾损伤患者中的疗效。方法 纳入河南省胸科医院2021年1月至2022年12月收治的100例老年糖尿病合并冠心病伴肾损伤患者作为研究对象,采用1∶1随机分组的方式将纳入的患者分为两组,各50例。对照组接受沙库巴曲缬沙坦治疗,观察组接受沙库巴曲缬沙坦结合达格列净治疗。对比两组患者心功能(左心室射血分数、左心室收缩末期容积、左心室舒张末期容积、左心室舒张末内径)、血糖指标(空腹血糖、餐后2 h血糖、胰岛β细胞功能指数、胰岛素抵抗指数)、炎症因子水平(γ-干扰素、白细胞介素-6、肿瘤坏死因子-α、降钙素原)、肾功能(肾小球滤过率、尿素氮、肌酐、肌酐清除率)。结果 用药后,观察组左心室射血分数、左心室收缩末期容积高于对照组,左心室舒张末期容积、左心室舒张末内径低于对照组(P<0.05)。肾功能比较,观察组尿素氮、肌酐、肾小球滤过率水平均低于对照组,肌酐清除率高于对照组(P<0.05),观察组空腹血糖、餐后2 h血糖以及胰岛素抵抗指数水平低于对照组,胰岛β细胞功能水平高于对照组(P<0.05),且用药后观察组γ-干扰素、白细胞介素-6、肿瘤坏死因子-α、降钙素原低于对照组(P<0.05)。结论 对老年糖尿病合并冠心病伴肾损伤患者给予沙库巴曲缬沙坦联合达格列净联合治疗,可提升患者的心功能、改善血清因子水平、缓解机体炎症。
Objective To analyze the efficacy of sarkubatrivalsartan combined with Dapagliflozin in elderly diabetes patients with coronary heart disease and renal injury.Methods A total of 100 elderly patients with diabetes combined with coronary heart disease and renal injury admitted to Henan Chest Hospital from January 2021 to December 2022 were observed.The patients were randomly divided into two groups by 1∶1,with 50 cases in each group.The control group received treatment with sacubitril valsartan,while the observation group received treatment with sacubitril valsartan combined with Dapagliflozin.The patient’s cardiac function(left ventricular ejection fraction,left ventricular end systolic volume,left ventricular end diastolic volume,left ventricular end diastolic diameter),blood glucose indicators(fasting blood glucose,2-hour postprandial blood glucose,pancreatic beta cell function index,insulin resistance index),inflammatory factor levels(interferon gamma,interleukin-6,tumor necrosis factor alpha,procalcitonin),and renal function(glomerular filtration rate,urea nitrogen,creatinine,creatinine clearance rate)were compared.Results After medication,the left ventricular ejection fraction and left ventricular end systolic volume in the observation group were higher than those in the control group,while the left ventricular end diastolic volume and left ventricular end diastolic diameter were significantly lower than those in the control group(P<0.05).Renal function comparison showed that the levels of urea nitrogen,creatinine,and glomerular filtration rate in the observation group were lower than those in the control group,while the creatinine clearance rate was higher than that in the control group(P<0.05).Fasting blood glucose,postprandial 2-hour blood glucose,and insulin resistance index levels in the observation group were lower than those in the control group,while pancreatic beta cell function levels were higher than those in the control group(P<0.05).After medication,the levels of interferon gamma,interleukin-6,tumor necrosis factor alpha,and procalcitonin in the observation group were lower than those in the control group(P<0.05).Conclusion The combination treatment of sarkubatrivalsartan combined with Dapagliflozin for elderly patients with diabetes combined with coronary heart disease and kidney injury can improve the heart function of patients,improve serum factor levels and alleviate body inflammation.
