摘要
目的 检索、评价和汇总成人心脏病患者住院期间氧疗管理的最佳证据。方法 根据“5S”证据金字塔模型,系统检索计算机决策系统、相关指南与专业协会网站及数据库中关于成人心脏病患者住院期间氧疗管理的证据,检索时限为2019年1月1日—2024年12月31日。结果 共纳入25篇文献,其中临床决策9篇、指南4篇、系统评价5篇、证据总结7篇,共形成35条证据,包括氧疗前准备(3条)、评估氧合状态(2条)、制定氧合目标(3条)、启动氧疗(8条)、监测氧疗效果(6条)、调整氧浓度(5条)、选择氧疗工具(6条)和终止氧疗(2条)8个方面。结论 本研究总结了成人心脏病患者住院期间氧疗管理的最佳证据,可为临床开展循证实践提供依据。医务人员在临床应用过程中,需根据当地医疗卫生水平、患者经济状况及自身意愿,制订个体化的氧疗管理方案。
Objective To retrieve,evaluate,and summarize the best evidence on oxygen therapy management for adult cardiac patients during hospitalization.Methods Based on the"5S"evidence pyramid model,a systematic search was conducted in computerized decision support systems,relevant guidelines,professional association websites,and databases for evidence on oxygen therapy management for adult cardiac patients during hospitalization.The search period spanned from January 1,2019,to December 31,2024.Results A total of 25 articles were included,comprising 9 clinical decision support documents,4 guidelines,5 systematic reviews,and 7 evidence summaries.These sources yielded 35 pieces of evidence,covering 8 aspects:pre-oxygenation preparation(3 items),assessment of oxygenation status(2 items),setting oxygenation targets(3 items),initiating oxygen therapy(8 items),monitoring oxygen therapy efficacy(6 items),adjusting oxygen concentration(5 items),selecting oxygen delivery devices(6 items),and discontinuing oxygen therapy(2 items).Conclusions This study summarizes the best evidence on oxygen therapy management for adult cardiac patients during hospitalization,providing a foundation for evidence-based clinical practice.Healthcare professionals should develop individualized oxygen therapy management plans based on local medical resources,patients'financial conditions,and personal preferences when applying these findings in clinical settings.
