摘要
目的 探讨胎儿泌尿生殖系统畸形筛查中三维与四维彩色多普勒超声(简称彩超)检查的临床价值。方法 将2021年1月—2023年6月菏泽市第六人民医院收治的疑似胎儿泌尿生殖系统先天畸形孕产妇80例纳入研究,所有患者均进行三维和四维彩超检查,以磁共振成像(magnetic resonance imaging,MRI)结果为金标准,比较三维和四维彩超的诊断结果及诊断效能。结果 80例疑似胎儿泌尿生殖系统先天畸形孕产妇中,MRI检出阳性46例,阴性34例;三维彩超检出阳性45例,阴性35例,Kappa值为0.567;四维彩超检出阳性47例,阴性33例,Kappa值为0.923。四维彩超的灵敏度为97.83%(45/46),高于三维彩超的80.43%(37/46),差异有统计学意义(χ^(2)=4.339,P<0.05)。四维彩超的特异度、准确度、阳性预测值、阴性预测值均高于三维彩超,差异均有统计学意义(P均<0.05)。结论相比三维彩超,四维彩超对于泌尿生殖系统畸形筛查具有更高的诊断价值,与MRI检查结果的一致性良好。
Objective To explore the clinical value of three-dimensional and four-dimensional color Doppler ultrasonography in the screening of fetal genitourinary system malformations.Methods From January 2021 to June 2023,80 pregnant women with suspected congenital malformations of fetal genitourinary system admitted to Heze Sixth People's Hospital were included in the study.All patients were examined by threedimensional and four-dimensional color Doppler ultrasound.The results of magnetic resonance imaging(MRI)were used as the gold standard to compare the diagnostic results and diagnostic efficacy of three-dimensional and four-dimensional color ultrasound.Results Among 80 pregnant women with suspected congenital malformation of fetal genitourinary system,46 cases were positive and 34 cases were negative detected by MRI.There were 45 positive cases and 35 negative cases detected by three-dimensional color Doppler ultrasound,and the Kappa value was 0.567.Four-dimensional color Doppler ultrasound detected 47 positive cases and 33 negative cases,and the Kappa value was 0.923.The sensitivity of four-dimensional color Doppler ultrasound was 97.83%(45/46),which was higher than 80.43%(37/46)of three-dimensional color Doppler ultrasound,and the difference was statistically significant(χ^(2)=4.339,P<0.05).The specificity,accuracy,positive predictive value and negative predictive value of four-dimensional color Doppler ultrasound were higher than those of three-dimensional color Doppler ultrasound,and the differences were statistically significant(all P<0.05).Conclusion Compared with three-dimensional color Doppler ultrasound,four-dimensional color Doppler ultrasound has higher diagnostic value for genitourinary system malformation screening,and has good consistency with MRI results.
