摘要
目的 探讨真武汤加减对阳虚水泛型心力衰竭患者利尿剂抵抗的治疗效果。方法 选择2021年1月—2024年6月涟水县中医院收治的76例阳虚水泛型心力衰竭伴利尿剂抵抗患者,按照治疗方法不同进行分组,对照组(n=38)予呋塞米治疗,观察组(n=38)在对照组基础上加用真武汤加减治疗,比较两组临床疗效、尿量与心功能以及炎症反应。结果 观察组治疗总有效率为92.11%(35/38)高于对照组的73.68%(28/38),差异有统计学意义(χ^(2)=4.547,P=0.033)。治疗后,观察组尿量、左室射血分数较对照组高,N末端B型利钠肽前体、超敏C反应蛋白、白细胞介素-6水平较对照组低,差异均有统计学意义(P均<0.05)。结论 对阳虚水泛型心力衰竭利尿剂抵抗患者予真武汤加减可改善排尿状态与心功能,减轻炎症反应。
Objective To investigate the therapeutic effect of Zhenwu Decoction on diuretic resistance in patients with yang-deficiency water-generalized heart failure.Methods A total of 76 patients with yangdeficiency water-generalized heart failure accompanied by diuretic resistance admitted to Lianshui County Hospital of Traditional Chinese Medicine from January 2021 to June 2024 were selected and divided into two groups according to different treatment methods.The control group(n=38)was treated with furosemide,while the observation group(n=38)was treated with modified Zhenwu Decoction on the basis of the control group.The clinical efficacy,urine volume and cardiac function indicators,the inflammatory response status of the body of the two groups were compared.Results The total effective rate of the observation group was 92.11%(35/38),which was higher than 73.68%(28/38)of the control group,and the difference was statistically significant(χ^(2)=4.547,P=0.033).After treatment,the urine output and left ventricular ejection fraction of the observation group were higher than those of the control group,while the levels of N-terminal B-type natriuretic peptide precursor,high-sensitivity C-reactive protein,and interleukin-6 were lower than those of the control group,and the differences were statistically significant(all P<0.05).Conclusion Resistance to diuretics in heart failure caused by yang deficiency water-generalized patients can be improved by reducing the use of Zhenwu Decoction,which can improve urinary status and heart function,and alleviate inflammatory reactions.
