儿科住院医师规范化培训多层评价体系的构建

摘要 目的探讨柯氏模型联合背景-输入-过程-结果评价(CIPP)模型的多层评价体系在儿科住院医师规范化培训中的应用效果。方法方法选取2020年9月-2023年6月期间我院在培的2018级、2019级、2020级共39名儿科住培学员为研究对象,构建柯氏模型联合CIPP模型的多层评价体系,采用问卷调查法了解住培学员培训过程中的自我感受及培训项目修正后的自我感受,比较培训前和培训后住培学员的成绩及学员自身、带教老师和同组同学对其工作能力的评价,并与2017级13名毕业学员的就业率、患者投诉率、结业考核通过率、医师资格考试通过率进行对比。结果结果住培学员培训后,对培训项目的师资、内容、方法、设施的满意度均高于培训过程中(均P<0.05)。住培学员培训后,理论知识考核、临床技能考核、客观结构化临床考试(OSCE)考核成绩均高于培训前(均P<0.05)。住培学员培训后,学员自身、带教老师和同组同学对其在人际沟通能力、知识技能应用能力、科研能力的工作能力评价得分均高于培训前(均P<0.05)。2018、2019、2020级住培学员的患者投诉率低于2017级毕业学员。结论结论柯氏模型联合CIPP模型的多层评价体系提高了医院儿科住培学员培训效果和儿科住培工作质量,为全国儿科住培工作提供了新的培训方法和模式。 Objective To explore the application effect of the multi-level evaluation system combining Kirkpatrick’s model with the Context-Input-Process-Product(CIPP)model in the standardized training of pediatric residents.Methods From September 2020 to June 2023,a total of 39 pediatric resident trainees from the 2018,2019,and 2020 cohorts in our hospital were selected as the subjects of the study.A multi-level evaluation system combining Kirkpatrick’s model and the CIPP model was constructed.A questionnaire survey was conducted to understand the trainees’self-perception during the training process and after the revision of the training program,and to compare the trainees’performance before and after the training,as well as the evaluations of their work ability by themselves,their instructors,and their peers,with the employment rate,patient complaint rate,graduation examination pass rate,and medical qualification examination pass rate of 13 graduates from the year 2017.Results The satisfaction of the trainees with the training program’s faculty,content,methods,and support facilities was higher after the training than during the training process(allP<0.05).The scores of the trainees in theoretical knowledge assessment,clinical skills assessment,and Objective Structured Clinical Examination(OSCE)were higher after the training than before(allP<0.05).The scores of the trainees’self-evaluation,instructors’evaluation,and peers’evaluation on interpersonal communication ability,knowledge and skill application ability,and research ability were all higher after the training than before(allP<0.05).The patient complaint rates of the trainees from the 2018,2019,and 2020 cohorts were lower than those of the graduates from the year 2017.Conclusion The multi-level evaluation system combining Kirkpatrick’s model and the CIPP model has improved the training effectiveness and work quality of pediatric resident trainees in hospitals,providing new training methods and models for pediatric resident training nationwide.
