摘要
本文总结1例强直性脊柱炎合并重度脊柱后凸畸形患者分期矫形围术期护理经验,重点关注呼吸系统、术中出血、血液灌注、体位改变及压力性损伤高风险的管理。护理要点:术前多学科(MDT)团队联合评估肺功能、制定应急方案及体位模拟;术中密切关注患者情况,时刻掌握生命体征变化、末梢血运情况、重要器官灌注情况,加强体温保护,积极预防压力性损伤等并发症;术后积极预防循环血容量不足及活动性出血,开展早期康复锻炼,落实延续护理措施。经多学科救治和精心护理,患者3次手术均顺利完成,术后恢复良好,均按期出院。
This paper summarized the nursing exper ience of a patient with ankylosing spondy⁃litis complicated with sever e kyphotic defor mity and or thopaedic Surgery in 3 stages,focusing on the management of respirator y system,intraoper ative bleeding,blood per fusion,position change and high risk of stress injury.The main nursing points are as follows:Participating in multidisci⁃plinary discussion befor e Surgery,deter mining the surgical plan and alter native plan,evaluating the patient's surgical preparation state and pulmonary function,communication with the clinic to improve the allocation of intraoper ative supplies and conducting posture simulation;During the oper ation,the traveling nurses closely monitor ed the changes of the patients'vital signs,per ipher⁃al blood transpor t,vital or gan per fusion,enhanced the intraoper ative body temper ature protection and prevention of pressure injuries.Comprehensive inter ventions including prevention of postop⁃er ative complications such as hypovolemia and active hemor rhage,functional exer cise,rapid reha⁃bilitation program and discharge education were carried out.After multidisciplinary treatment and careful nursing,the patient under went 3 successful oper ations,recover ed well after Surgery,and was discharged on schedule.
