摘要
目的:基于积极心理学理论构建非自杀性自伤(Non-Suicidal Self-Injury,NSSI)青少年积极心理团体干预方案并评价其干预效果。方法:通过文献查阅,基于NSSI心理质性研究的Meta整合结果构建干预方案初稿,并通过专家会议和预实验形成干预方案修订稿和实施稿。2023年4-9月,选取河南省某三级甲等医院NSSI青少年患者83例作为研究对象,随机分为两组。干预组在心理健康教育的基础上接受为期6周、每周2时/次的积极心理团体干预。采取渥太华自我伤害调查量表(OSI)、积极消极情绪量表(PANAS)、一般自我效能感量表(GSES)及脑功能核磁共振成像(fMRI)对患者进行评估,最终70例患者完成本研究并纳入分析。结果:干预后,两组OSI总分、积极消极情绪总分以及自我效能感总分存在统计学差异(t=19.650,32.965,6.175,3.304;P<0.001),干预组静息脑功能核磁共振成像(fMRI)额叶区脑功能活跃度明显降低,存在统计学差异(P<0.001)。结论:积极心理学理论的团体干预方案,有利于减轻NSSI青少年的自伤行为,减轻消极情绪,增加积极情绪和自我效能感,改善额叶区脑功能。
Objective:To construct a group intervention program for adolescents with non-suicidal self-injury(NSSI)based on positive psychology theory and evaluate its application effect.Methods:First draft of intervention program was constructed based on the meta-integration results of the qualitative study of NSSI;Revision and implementation drafts were formed through expert meeting and pilot study.83 adolescents with NSSI from a tertiary hospital in Zhengzhou between April 2023 and September 2023 who met the inclusion criteria were randomly divided into two groups.The intervention group received a 6-week positivepsychological group intervention,conducted twice a week for 2 hours per session,in addition to routine mental health education.The Ottawa Self-Injury Inventory(OSI),Positive and Negative Emotions Scale(PANAS),General Self-Efficacy Scale(GSES)and brain functional magnetic resonance imaging(fMRI)were used to evaluate the effects of the two groups before and after the intervention.Finally,a total of 70 participants completed this study and were included in analysis.Results:After intervention,total score of the OSI,positive and negative emotion and self-efficacy(t=19.650,32.965,6.175,3.304;P<0.001)had significantly differences.Brain functional magnetic resonance imaging(fMRI)in the intervention group showed that the brain function activity in the frontal lobe region was significantly lower than that before the intervention(P<0.001).Conclusion:The group intervention program based on positive psychology theory effectively reduces self-harming behaviors in adolescents with NSSI,alleviates negative emotions,enhances positive emotions and self-efficacy,and improves frontal lobe brain function.
