摘要
目的:探讨脊髓损伤后神经源性膀胱患者应用中药敷脐的效果。方法:择取河南省郑州市骨科医院106例脊髓损伤后神经源性膀胱患者为研究对象,随机分组。对照组53例给予膀胱功能训练、经皮胫神经电刺激,观察组53例在对照组基础上增加中药敷脐。4周后,比较2组患者症状改善情况、膀胱功能、尿流动力学指标。结果:干预后,观察组国际下尿路症状评分量表(LUTS)、泌尿症状困扰评分(USDS)分值低于对照组(P<0.05);干预后,观察组日均导尿次数低于对照组,日均单次排尿量、日均排尿次数高于对照组(P<0.05);干预后,观察组最大尿流率、最大膀胱容量、充盈期逼尿肌压力、最大尿道闭合压高于对照组(P<0.05)。结论:脊髓损伤后神经源性膀胱患者在接受膀胱功能训练、经皮胫神经电刺激的基础上,增加中药敷脐能够有效减轻其临床症状,改善膀胱功能,提高控尿能力。
Objective:To explore the effect of applying Chinese medicine to the navel of patients with neurogenic bladder after spinal cord injury.Methods:A total of 106 patients with neurogenic bladder after spinal cord injury in Zhengzhou Orthopedic Hospital of Henan Province were selected as the research subjects and randomly divided into two groups.53 cases in the control group were given bladder function training and transcutaneous tibial nerve electrical stimulation,and 53 cases in the observation group were given Chinese medicine to the navel on the basis of the control group.After 4 weeks,the symptom improvement,bladder function and urodynamic indexes of the two groups were compared.Results:After intervention,the international lower urinary tract symptom rating scale(LUTS)and urinary symptom distress score(USDS)scores of the observation group were lower than those of the control group(P<0.05);after intervention,the average daily catheterization times of the observation group were lower than those of the control group,and the average daily single urination volume and average daily urination times were higher than those of the control group(P<0.05);after intervention,the maximum urine flow rate,maximum bladder capacity,filling detrusor pressure and maximum urethral closure pressure of the observation group were higher than those of the control group(P<0.05).Conclusion:In addition to bladder function training and transcutaneous tibial nerve electrical stimulation,Chinese medicine navel application can effectively alleviate clinical symptoms,improve bladder function,and enhance urinary control ability in patients with neurogenic bladder after spinal cord injury.
