摘要
目的 观察舒肝解郁胶囊联合氟哌噻吨美利曲辛治疗难治性良性阵发性位置性眩晕(BPPV)的临床效果。方法 选取2022年1月—2023年6月石河子市人民医院接收的难治性BPPV患者70例,依据随机数字表法分为观察组和对照组,每组35例。2组均给予前庭康复训练,对照组给予氟哌噻吨美利曲辛治疗,观察组在对照组治疗的基础上给予舒肝解郁胶囊治疗,2组均持续治疗12周。比较2组治疗前、治疗后1个月、3个月、6个月脑血流循环指标[基底动脉(BA)、左椎动脉(LVA)、右椎动脉(RVA)血流速度]、视觉模拟评分法(VAS)评分、眩晕残障程度量表(DHI)评分、平衡信心量表(ABC)评分、汉密尔顿焦虑量表(HAMA)评分、汉密尔顿抑郁量表(HAMD)评分,复发率。结果 治疗后1个月、3个月、6个月,2组BA、LVA及RVA血流速度均较治疗前增加,且观察组增加幅度大于对照组(P<0.01);2组VAS、DHI、HAMA、HAMD评分均较治疗前降低,ABC评分均较治疗前升高,且观察组改善幅度大于对照组(P<0.05或P<0.01)。治疗后6个月,观察组复发率为2.86%(1/35),低于对照组的17.14%(6/35)(χ^(2)=3.968,P=0.046)。结论 舒肝解郁胶囊联合氟哌噻吨美利曲辛治疗难治性BPPV可改善患者的脑血流循环指标,减轻焦虑抑郁情绪,降低眩晕复发率。
Objective To observe the clinical effect of Shugan Jieyu capsules combined with flupentixol and melitracen in the treatment of refractory benign paroxysmal positional vertigo(BPPV).Methods A total of 70 patients with refractory BPPV admitted to Shihezi People′s Hospital from January 2022 to June 2023 were selected and divided into observation group and control group according to random number table method,with 35 cases in each group.Both groups were given vestibular rehabilitation training,the control group was given flupentixol and melitracen treatment,and the observation group was given Shugan Jieyu capsules treatment on the basis of the control group treatment.Both groups were treated for 12 weeks.The cerebral blood flow index(blood flow velocity of BA,LVA and RVA),VAS,DHI,ABC,HAMA,HAMD scores before treatment,1 month,3 months and 6 months after treatment,recurrence rate were compared between the two groups.Results At 1 month,3 months and 6 months after treatment,the blood flow velocity of BA,LVA and RVA in two groups were increased compared with that before treatment,and the increase rate in observation group was greater than that in control group(P<0.01).VAS,DHI,HAMA and HAMD scores in two groups were lower than before treatment,ABC scores were higher than before treatment,and the improvement of observation group was greater than that of control group(P<0.05 or P<0.01).At 6 months after treatment,the recurrence rate of observation group was 2.86%(1/35),which was lower than that of control group(17.14%,6/35)(χ^(2)=3.968,P=0.046).Conclusion Shugan Jieyu capsules combined with flupentixol melitripen in the treatment of refractory BPPV can improve cerebral blood flow index,relieve anxiety and depression,and reduce the recurrence rate of vertigo.
