薄层液基细胞学检查联合DNA倍体分析及HPV检测在宫颈上皮内病变筛查中的应用价值

The Role of Thin-Layer Liquid-Based Cytology Combined with DNA Ploidy Analysis and HPV Detection in the Screening of Cervical Eepithelial Lesions

摘要 目的探讨薄层液基细胞学(TCT)检查联合DNA倍体分析及HPV检测在宫颈上皮性病变筛查中的应用价值,为早期宫颈癌的筛查提供可靠依据。方法回顾性统计2020年6月—2024年8月在北京市第一中西医结合医院进行宫颈检查的符合条件的13652例患者的检查结果,所有患者均行TCT、DNA及HPV检查,并对可疑病例行宫颈活检检查,采用ANOVA(方差分析)方法计算组间及组内P值。结果既有DNA倍体扩增又有HPV感染的宫颈活检病例中NILM有1例(1.3%)为CINⅡ+;ASC-US有18例(2.89%);ASC-H有4例(4.88%);LSIL有20例(5.36%);HSIL+有22例(32.35%)(包含3例鳞状细胞癌);CINⅡ+占比随TCT病变程度逐级增高,组内差异有统计学意义(P<0.05),且在TCT各级病变中,同时伴DNA倍体扩增及HPV感染的病例均比伴DNA扩增不伴HPV感染、不伴DNA扩增伴HPV感染及不伴DNA扩增且不伴HPV感染者具有更高的CINⅡ+检出率,组间差异有统计学意义(P<0.05)。结论宫颈TCT检查联合DNA倍体分析及HPV检测,有助于提高宫颈上皮性病变的检出率,为宫颈癌的早期筛查提供较准确的方法。 Objective To explore the application value of thinprep cytology test(TCT)combined with DNA ploidy analysis and HPV detection in the screening of cervical epithelial lesions,and to provide a reliable basis for the screening of early cervical cancer.Methods A retrospective analysis of 13652 eligible patients collected in Beijing First Hospital of Integrated Traditional Chinese and Western Medicine from June 2020 to August 2024 was performed.All patients underwent TCT,DNA and HPV tests,and cervical biopsy was performed on suspected cases.ANOVA(analysis of variance)method was used to calculate P values between groups and within groups.Results Among the cervical biopsy cases with both DNA ploidy amplification and HPV infection,1 case(1.3%)of NILM was CINII+;there were 18 cases(2.89%)of ASC-US;there were 4 cases(4.88%)of ASC-H;there were 20 cases(5.36%)of LSIL;there were 22 cases(32.35%)of HSIL+(including 3 cases of squamous cell carcinoma);the proportion of CINII+increased gradually with the degree of TCT lesions,and the difference was statistically significant(P<0.05).In all levels of TCT lesions,the cases with DNA ploidy amplification and HPV infection had higher CINII+detection rate than those with DNA amplification without HPV infection,without DNA amplification with HPV infection and without DNA amplification and without HPV infection.The difference between the groups was statistically significant(P<0.05).Conclusion Cervical TCT combined with DNA ploidy analysis and HPV detection is helpful to improve the detection rate of cervical epithelial lesions and provide a more accurate method for early screening of cervical cancer.
