摘要
目的 分析医院介入放射工作人员外周血淋巴细胞染色体畸变和微核异常情况及影响因素,为介入放射防护与相关标准完善提供依据。方法 分层随机抽取2021年重庆市开展介入放射诊疗的医院17家,将长期、低剂量接触辐射的622名介入放射工作人员作为研究对象,采集人员基本信息、个人剂量监测结果以及外周血淋巴细胞染色体畸变和微核监测数据进行比较和多因素logistic回归分析。结果 三级医院介入放射工作人员年有效剂量均值为0.31(0.18,0.55)mSv,二级医院介入放射工作人员年有效剂量均值为0.27(0.11,0.45)mSv。介入放射工作人员染色体畸变异常率为2.73%,微核异常率8.84%;岗位的染色体畸变(χ^(2)=6.408,P=0.041)和微核(χ^(2)=11.423,P=0.003)异常率由低到高,依次为护士、技师和医师,差异均有统计学意义;三级医院微核异常率高于二级医院,差异有统计学意义(χ^(2)=3.944,P=0.047);染色体畸变(χ^(2)=9.538,P=0.023)和微核(χ^(2)=11.554,P=0.009)异常率随年龄越大,异常率越高,差异均有统计学意义;多因素logistic回归分析显示,年有效剂量是染色体畸变(P<0.05,OR=1.303,95%CI:1.062~1.599)和微核(P<0.05,OR=1.901,95%CI:1.386~2.608)异常的危险因素,年有效剂量越高,异常率越大。结论 重庆市介入放射工作人员的染色体畸变和微核异常率较高,针对性地做好工作防护,降低年有效剂量,可有效保护介入放射人员的健康。
Objective To analyze the chromosomal aberrations and micronucleus abnormalities in peripheral blood lymphocytes of interventional radiology workers in the hospitals and the influencing factors,and to provide a basis for improving interventional radiological protection and related standards.Methods A stratified random sampling method was used to select 17 hospitals in Chongqing Municipality that carried out interventional radiology diagnosis and treatment in 2021,and a total of 622 interventional radiology workers who were exposed to long-term and low-dose radiation served as the research subjects.We collected the data about the subjects’general information,individual dose monitoring results,peripheral blood lymphocyte chromosome aberrations and micronucleus monitoring for difference comparison and multivariate logistic regression analysis.Results The average annual effective dose of workers engaging in interventional radiology in tertiary hospitals was 0.31(0.18,0.55)mSv,and that of interventional radiology workers in secondary hospitals was 0.27(0.11,0.45)mSv.The abnormal rates of chromosome aberration and micronuclei in the interventional radiation workers were 2.73%and 8.84%respectively.Among the interventional radiology workers in different job positions,the abnormal rates of chromosome aberrations(χ^(2)=6.408,P=0.041)and micronuclei(χ^(2)=11.423,P=0.003)were found to be the lowest in nurses,followed by technicians and physicians,showing statistically significant differences.The abnormal rate of micronuclei was higher in tertiary hospitals than in secondary hospitals,with a statistically significant difference(χ^(2)=3.944,P=0.047).The abnormal rates of chromosome aberrations(χ^(2)=9.538,P=0.023)and micronuclei(χ^(2)=11.554,P=0.009)increased with age,showing statistically significant differences.Multivariate logistic regression analysis displayed that the annual effective dose was a risk factor for chromosomal aberrations ( P< 0.05,OR = 1.303,95%CI: 1.062-1.599) and micronucleus abnormalities ( P< 0.05,OR = 1.901,95%CI: 1.386- 2.608) . The higher the annualeffective dose,the higher the abnormal rate. Conclusion The abnormal rates of chromosome aberrations and micronuclei amongthe interventional radiology workers in Chongqing Municipality were relatively high. Targeted occupational protection and reductionof the annual effective dose can effectively protect the interventional radiology workers’health.
