摘要
肺痹最早见于《素问·痹论》“肺痹者,烦满喘而呕”,用于描述痹之客肺脏者,由于历代医家病名编撰格式及用词习惯的不同,肺痹作为疾病病名一直未得到规范使用。直至2023年版《中医临床诊疗术语第1部分:疾病》明确肺痹是以喘促憋闷,胸膈胀满,动辄尤甚,干咳、少痰等为特征的持续进行性肺系疾病。但目前肺痹作为疾病病名在使用上仍存在“病”“证”“症”界定不清及中西病名混杂的现象。笔者查阅文献,总结出以症为名、概念混淆、阐述不明是造成其标准化不足的原因。基于此,笔者对肺痹、肺气郁痹、肺络痹阻等进行梳理,建议肺痹应作为间质性肺疾病的中医病名,以期为中医肺痹病名规范化提供参考。
The term"Lung bi"first appeared in SuWen·Bi Lun,where it was described as"Lung bi is characterized by irritability,fullness,panting,and vomiting,"referring to pathological conditions caused by Bi syndrome invading the lung.Due to differences in terminology formats and language habits among medical scholars throughout history,"Lung bi"has not been consistently used as a standardized disease name.It was not until the 2023 edition of TCM Clinical Diagnostic and Therapeutic Terminology Part l:Diseases that"Lung bi"was clearly defined as a persistent progressive pulmonary disease characterized by shortness of breath,oppression in the chest,distension and fullness in the thorax,worsening with exertion,dry cough,and minimal sputum production.However,the use of"Lung bi"as a disease name still suffers from unclear distinctions between disease,syndrome,and symptom,as well as the mixing of TCM and Western disease names.After reviewing relevant literature,the authors found that naming based on symptoms alone,conceptual confusion,and ambiguous descriptions are the main reasons for the lack of standardization.Based on this analysis,the authors systematically review related terms such as Lung bi,Lung bi qi stagnation,and Lung bi collateral obstruction,and suggest that""Lung bi"should be adopted as the TCM disease name corresponding to interstitial lung disease in western medicine,aiming to provide a reference for the standardization of the disease name"Lung bi"in traditional Chinese medicine.
