摘要
目的探讨蔗糖铁溶液静脉注射对冠心病(CHD)合并慢性心力衰竭(CHF)患者伴铁缺乏的干预效果。方法选取2021年7月至2023年12月新余市人民医院收治的80例CHD合并CHF患者作为研究对象,按照随机数字表法分为对照组和观察组,每组40例。对照组口服右旋糖酐铁口服溶液治疗,观察组静脉注射蔗糖铁溶液治疗,比较两组患者的6 min步行试验(6-MWT)距离、心功能、铁代谢指标、不良事件以及预后。结果治疗3个月后,观察组6-MWT距离长于对照组,左心室射血分数(LVEF)、血清铁(SI)、转铁蛋白饱和度(TS)、血清铁蛋白(SF)、总铁结合力(TIBC)高于对照组,N端脑钠肽前体(NT-proBNP)低于对照组,差异有统计学意义(P<0.05)。两组患者不良事件总发生率、再住院率、死亡率比较,差异无统计学意义(P>0.05)。结论静脉注射蔗糖铁溶液可改善CHD合并CHF患者伴铁缺乏的体内铁代谢情况和心功能,提高患者运动耐量,有利于减少治疗过程中不良事件和不良预后的发生。
Objective To explore the intervention effect of intravenous injection of Iron Sucrose Solution in patients with coronary heart disease(CHD)complicated with chronic heart failure(CHF)accompanied by iron deficiency.Methods Eighty patients with CHD combined with CHF admitted to Xinyu People's Hospital from July 2021 to December 2023 were selected as the research objects,and they were divided into the control group and the observation group according to the random number table method,with 40 cases in each group.The control group was treated with Iron Dextran Oral Solution,while the observation group was treated with intravenous injection of Iron Sucrose Solution.The 6-minute walk test(6-MWT)distance,cardiac function,iron metabolism indicators,adverse events and prognosis of the two groups of patients were compared.Results After 3 months of treatment,the 6-MWT distance in the observation group was longer than that in the control group,the left ventricular ejection fraction(LVEF),serum iron(SI),transferrin saturation(TS),serum ferritin(SF),and total iron binding capacity(TIBC)were higher than those in the control group,while the N-terminal pro-brain natri uretic peptide(NT-proBNP)was lower than that in the control group,the differences were statistically significant(P<0.05).There were no statistically significant differences in the total incidence of adverse events,rehospitalization rate and mortality rate between the two groups of patients(P>0.05).Conclusion Intravenous injection of Iron Sucrose Solution can improve the iron metabolism and cardiac function in patients with CHD combined with CHF accompanied by iron deficiency,enhance the exercise tolerance of patients,and is con ducive to reducing the occurrence of adverse events and adverse prognosis during the treatment process.
