摘要
目的:探究中药熏蒸联合活血解毒汤治疗斑块型银屑病的效果及机制。方法:采用随机数字表法将石家庄市中医院2021年1月—2024年1月收治的158例斑块型银屑病患者分为治疗组和对照组,每组79例。对照组接受消银胶囊和复方甘草酸苷胶囊治疗,治疗组接受中药(消银液)熏蒸配合活血解毒汤治疗。治疗8周后,比较两组疗效、银屑病面积与严重指数(PASI)积分、血清白细胞介素(IL)-22、IL-17A水平变化、中医证症状积分以及不良反应发生情况。结果:治疗组总有效率显著高于对照组(P<0.05);两组患者治疗前、治疗2周后、治疗4周后、治疗6周后、治疗8周后的PASI积分、IL-22、IL-17A组间、时间、交互效应比较差异有统计学意义(P<0.05),治疗组中医证候积分低于对照组(P<0.05),两组不良反应比较差异无统计学意义(P>0.05)。结论:中药熏蒸疗法联合活血解毒汤加减治疗斑块型银屑病能显著提高临床疗效,改善患者皮损情况,改善血清IL-22、IL-17A水平。
Objective:To investigate the efficacy and mechanism of Chinese Herbal Fumigation combined with Huoxue Jiedu Decoction in treating plaque psoriasis.Methods:A total of 158 patients with plaque psoriasis admitted to Shijiazhuang Hospital of Traditional Chinese Medicine from January 2021 to January 2024 were randomly divided into a treatment group and a control group using a random number table,with 79 cases in each group.The control group received Xiaoyin Capsules and Compound Glycyrrhizin Capsules,while the treatment group received Chinese Herbal Fumigation(Xiaoyin Solution)combined with Huoxue Jiedu Decoction.After 8 weeks of treatment,the efficacy,Psoriasis Area and Severity Index(PASI)scores,serum levels of interleukin(IL)-22 and IL-17A,TCM syndrome scores,and incidence of adverse reactions were compared between the two groups.Results:The total effective rate in the treatment group was significantly higher than that in the control group(P<0.05).There were statistically significant differences in PASI scores,IL-22,IL-17A levels,time,and interaction effects between the two groups at 2,4,6,and 8 weeks after treatment,with statistical difference(P<0.05).The TCM syndrome scores in the treatment group were lower than those in the control group(P<0.05),and there was no significant difference in adverse reactions between the two groups(P>0.05).Conclusion:Chinese Herbal Fumigation combined with Huoxue Jiedu Decoction significantly improves clinical efficacy,alleviates skin lesions,and modulates serum IL-22 and IL-17A levels in patients with plaque psoriasis.
