摘要
本文主要总结孙占学教授治疗带状疱疹的中医经验。孙教授基于“毒邪”“络脉”认为带状疱疹分为早期和后神经痛期。若因饮食不节、家有变故、情志失调、气候环境等致素体正虚,正不胜邪,毒犯经络发为带状疱疹早期;毒邪稽留、气血失和、病络不复则发为后神经痛期。孙教授提出带状疱疹早期的主要病机特点为毒邪炽盛,治疗当以清热解毒止痛为主;后神经痛期以络损、络瘀、络虚、络阻为主要特点,当解毒护络、化瘀通络、养血活络、疏经通络。带状疱疹治疗期间也需要结合外治疗法和日常调护,方可得到最佳的治疗效果,缓解难忍之痛。
This paper summarizes Professor Sun Zhanxue's experience in treating herpes zoster with traditional Chinese medicine(TCM).Professor Sun classifies herpes zoster into early stage and postherpetic neuralgia based on the concepts of“toxic pathogens”and“collaterals.”In the early stage,if the patient has a constitution weakened by irregular diet,family disturbances,emotional disorders,or environmental factors,leading to a deficiency of healthy qi that fails to counteract the pathogenic toxins,the disease manifests as early-stage herpes zoster.If the toxic pathogens are retained,resulting in qi and blood disharmony and unhealed collaterals,the disease progresses to postherpetic neuralgia.Professor Sun suggests that the main pathogenic characteristics in the early stage are intense toxic heat.Therefore,the treatment should focus on clearing heat,detoxifying,and relieving pain.In the postherpetic neuralgia stage,the main characteristics are collateral damage,collateral stasis,collateral deficiency,and collateral obstruction.Treatment should include detoxifying and protecting collaterals,resolving stasis and unblocking collaterals,nourishing blood and invigorating collaterals,and freeing the meridians and unblocking collaterals.During the treatment of herpes zoster,it is also necessary to incorporate external treatments and daily care to achieve the best therapeutic effect and alleviate severe pain.
