QCC在提高抗菌药物治疗前病原学送检率的应用效果

摘要 目的 探讨运用品管圈(QCC)在某肿瘤专科医院提高医院住院患者抗菌药物治疗前病原学送检率的应用效果。方法运用QCC质量管理工具,通过主题选定、计划拟定、现况把握等标准活动步骤对医院提高住院患者抗菌药物治疗前病原学送检率开展QCC活动,分析送检率低的主要原因,制定有针对性的措施并实施改进,比较QCC活动前后提高医院住院患者抗菌药物治疗前病原学送检率的情况。结果 对策实施前期(2023年1-4月)、对策实施后期(2023年5-8月)和QCC整体活动期(2023年1-8月)抗菌药物治疗前病原学送检率分别为54.41%、68.64%和61.11%,较2022年1-8月送检率44.42%显著升高,差异有统计学意义(c^(2)=71.47、359.32、248.19,P<0.001)。对策实施前后医院感染诊断相关病原学送检率和联合使用重点药物前病原学送检率两个指标,2023年1-8月较2022年1-8月均有明显升高,差异有统计学意义(c^(2)=41.25、89.67,P<0.001)。结论 通过开展QCC活动,在有效提高住院患者抗菌药物治疗前病原学送检率的同时,圈员们各方面的能力也得到了锻炼和提升。 Objective To explore the application effect of quality control Circle(QCC)in improving the rate of pathogenic detection of inpatients before antibiotic treatment in a cancer hospital.Method The quality management tool of Quality Control Circle(QCC)was used to conduct QCC activities at the hospital to improve the pre-antimicrobial treatment pathogen sampling rate for inpatients.This involved standard activity steps such as theme selection,planning and current situation analysis.The main influencing factors for the low sampling rate were analyzed,targeted measures were developed and implemented for improvement,and pre-antimicrobial treatment pathogen sampling rate for inpatients at the hospital was compared before and after the QCC activity.Result The pathogen sampling rate before antibiotic treatment in the early stage(January to April in 2023),in the late stage(May to August in 2023)and in the overall active period of QCC(January to August in 2023)were 54.41%,68.64%and 61.11%,respectively,which were significantly higher than the rates of 44.42%between January 2022 and August 2022(x^(2)=71.47,359.32,248.19,P<0.001).Before and after the implementation of the countermeasures,the pathogenic detection rate related to the diagnosis of nosocomial infection and the pathogenic detection rate prior to the combination of key drugs significantly increased from January to August in 2023 compared to the same period in 2022,with statistical significance(x^(2)=41.25,89.67,P<0.001).Conclusion Conducting QCC activities can effectively improve the rate of pathogenic sampling rate of inpatients before antibiotic treatment,and the ability of the circle members will also be trained and improved.
