摘要
总结儿童抽动障碍(TD)从脏腑病位进行辨证和治疗的文献。认为其病位在肝,与心、脾、肺、肾等多个脏腑相关,风痰胶结、肝亢风动为主要病机,临床表现多样,但总以平肝息风为主要治则。根据所涉及脏腑的阴阳虚实而辨证处方加减。中医药治疗儿童TD,重视整体观念,强调辨证论治,不仅息风、清热、祛痰等以治其标,更应重视滋肾、补脾、柔肝等以治其本,脏腑病位辨证的多样性为临床提供了更多的治疗思路。
Tic disorder(TD)is a neuropsychiatric condition that primarily affects children and adolescents.This article reviews recent domestic literature on the syndrome differentiation and treatment of pediatric TD based on visceral pathogenesis.It is proposed that the primary pathological location of TD in children is the liver,with wind-phlegm entanglement and liver hyperactivity stirring wind as the main pathogenesis.Although clinical manifestations vary,the fundamental treatment principle remains calming the liver and extinguishing wind.Additionally,multi-viscera syndrome differentiation theories involving the kidney,heart,lung,and brain provide alternative perspectives for accurately determining pathological locations,thereby facilitating targeted clinical prescriptions.Nevertheless,the core treatment strategy still focuses on extinguishing wind and relieving convulsions.In the treatment of pediatric TD,traditional Chinese medicine(TCM)emphasizes a holistic approach and syndrome differentiation.It not only addresses symptoms by extinguishing wind,clearing heat,and resolving phlegm but also focuses on root causes by nourishing the kidney,strengthening the spleen,and soothing the liver.The diversity of visceral syndrome differentiation provides a broader range of clinical therapeutic strategies.
