摘要
目的对成人难治性支原体肺炎(RMPP)患者采用甲泼尼龙琥珀酸钠辅助联合支气管镜灌洗治疗,并对其治疗效果进行分析,以期能为该疾病患者寻找到安全、有效的治疗方案。方法选取2022年1月至2023年2月新疆医科大学第一附属医院收治的RMPP患者114例为研究对象,按数表法随机分为对照组与观察组,每组57例。入院后均对2组采取常规治疗,在此基础上,2组实施不同的治疗方案,对照组的治疗方案为添加甲泼尼龙琥珀酸钠,观察组的治疗方案为添加甲泼尼龙琥珀酸钠联合支气管镜灌洗,观察2组患者治疗前后的血清炎性因子、免疫功能、肺通气水平、胸肺顺应性的变化情况,并进行组间比较。结果2组患者经过治疗后,血清炎性因子[C反应蛋白(CRP)、降钙素原(PCT)、肿瘤坏死因子-α(TNF-α)]均比同组治疗前降低,而观察组治疗后的血清炎性因子水平均低于对照组(P<0.05);免疫功能(CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)水平)均比同组治疗前有所改善,而观察组治疗后的CD4^(+)、CD4^(+)/CD8^(+)水平均高于对照组,CD8^(+)则低于对照组(P<0.05);各项肺通气指标水平均比同组治疗前升高,而观察组则高于对照组(P<0.05);胸肺顺应性(胸廓顺应性、肺顺应性、总顺应性)均比同组治疗前升高,而观察组则高于对照组(P<0.05)。结论在RMPP患者的治疗中,在常规治疗的基础上添加甲泼尼龙琥珀酸钠辅助联合支气管镜灌洗治疗,对其炎性反应、胸肺顺应性、免疫功能、肺通气水平均具有良好的改善作用。
Objective To study the effect of bronchoscopic lavage combined with methylprednisolone sodium succinate in adult patients with refractory mycoplasma pneumoniae pneumonia(RMPP),aiming to find a safe and effective therapeutic solution.Methods The subjects of this study included 114 patients with RMPP who admitted to the First Affiliated Hospital of Xinjiang Medical University(January 2022-February 2023).They were divided into the control group(n=57)and observation group(n=57)by a random number table.All patients were managed by routine treatments and methylprednisolone sodium succinate,those in the observation group additionally bronchoscopic lavage.The changes in serum inflammatory factors,immune function,pulmonary ventilation level,and chest-lung compliance were compared between groups before and after treatment.Results After treatment,the levels of C-reactive protein(CRP),procalcitonin(PCT),tumor necrosis factor-α(TNF-α)were significantly lower in both groups than those before treatment,and the decreases in the observation group were more pronounced(P<0.05).The significantly higher CD4^(+),CD4^(+)/CD8^(+),and lower CD8^(+)were found in the both groups than those before treatment,and the improvements in the observation group were more common(P<0.05).The indicators of all pulmonary ventilation were significantly elevated compared to those before treatment,and the observation group showed higher levels than the control group(P<0.05).The chest-lung compliances including thoracic compliance,lung compliance,total compliance were all significantly increased compared to before treatment,with higher compliances in the observation group(P<0.05).Conclusion Bronchoscopic lavage combined with methylprednisolone sodium succinate and routine treatments have a beneficial effect on improving inflammatory response,chest-lung compliance,immune function,and pulmonary ventilation levels in patients with RMPP,with a good effect.
