摘要
目的探讨双侧丘脑底核电刺激(STN-DBS)联合小剂量多巴丝肼治疗帕金森病疗效及对抑郁症状、相关脑功能连接(FC)的影响。方法选取80例帕金森病患者,随机均分为对照组(多巴丝肼治疗)和观察组(STN-DBS联合多巴丝肼治疗)各40例。比较两组帕金森病症状严重程度(MDS-UPDRS)、汉密尔顿抑郁量表(HAMD)评分、匹茨堡睡眠质量指数(PSQI)评分、FC及不良反应情况。结果与治疗前比较,治疗后两组MDS-UPDRS各项评分、HAMD、PSQI评分均降低,各区zFC值均升高,且治疗后观察组较对照组趋势更明显(P<0.05)。观察组总不良反应发生率较对照组低(P<0.05)。结论STN-DBS联合小剂量多巴丝肼治疗帕金森病患者,可提高临床疗效,改善患者抑郁症状及脑功能异常连接情况。
Aim To investigate the efficacy of bilateral subthalamic nucleus-deep brain stimulation(STN-DBS)combined with low-dose levodopa-benserazide in the treatment of Parkinson’s disease(PD)and its effects on depressive symptoms and related brain functional connectivity(FC).Methods A total of 80 PD patients were enrolled and randomly divided into a control group(levodopa-benserazide treatment alone,n=40)and an observation group(bilateral STN-DBS combined with levodopa-benserazide,n=40).The severity of PD symptoms(MDS-UPDRS),Hamilton depression rating scale(HAMD)scores,Pittsburgh sleep quality index(PSQI)scores,FC,and adverse reactions were compared between the two groups.Results Compared with baseline,both groups showed reductions in MDS-UPDRS scores,HAMD scores,and PSQI scores,along with increased zFC values in various brain regions after treatment.Furthermore,these improvements were more pronounced in the observation group than in the control group(P<0.05).The total incidence of adverse reactions was lower in the observation group than in the control group(P<0.05).Conclusion The combination of bilateral STN-DBS and low-dose levodopa-benserazide in PD patients can enhance clinical efficacy,improve depressive symptoms,and normalize abnormal brain functional connectivity.
