摘要
目的探究脑小血管病(CSVD)患者定量脑电图(QEEG)特征与认知、抑郁症状的相关性。方法纳入CSVD患者96例为观察组,并选取年龄匹配的健康对照者88例。比较两组MRI、定量脑电图特征、抑郁情况[蒙哥马利-阿斯伯格抑郁评定量表(MADRS)评分]、认知功能[蒙特利尔认知评估(MoCA)量表评分和简易精神状态检查(MMSE)量表评分]。通过Logistic回归分析脑电图特征与认知、抑郁症状的相关性。结果与对照组相比,观察组抑郁MADRS评分升高,认知MMSE评分、MoCA评分降低,前额区及枕区θ波活动增多,振幅范围增高(P<0.05)。MRI显示观察组存在白质高信号、腔隙性梗死及微出血等典型脑小血管病改变。Logistic回归分析表明,θ波与MADRS评分呈正相关,与MoCA评分呈负相关。模型对抑郁症状评分和认知功能评分的解释力较强。结论脑小血管病患者的脑电图慢波出现频率与认知功能障碍、抑郁症状呈正相关。
Aim To explore the correlation between quantitative electroencephalogram(QEEG)characteristics and cognitive and depressive symptoms in patients with cerebral small vessel disease(CSVD).Methods A total of 96 CSVD patients were enrolled as the observation group,and 88 age-matched healthy control subjects were selected.MRI findings,QEEG characteristics,depressive symptoms(Montgomery-Asberg depression rating scale(MADRS)),and cognitive function(Montreal cognitive assessment(MoCA),minimental state examination(MMSE))were compared between the two groups.Logistic regression analysis was performed to assess the correlation between QEEG characteristics and cognitive and depressive symptoms.Results Compared with the control group,the observation group showed increased MADRS scores for depression,decreased MMSE and MoCA scores for cognition,enhancedθwave activity in the prefrontal and occipital region,and increased theta amplitude(P<0.05).MRI revealed typical CSVD changes,including white matter hyperintensities,lacunar infarcts,and microbleeds.Logistic regression analysis indicated that theta wave activity was positively correlated with MADRS scores and negatively correlated with MoCA scores.The model demonstrated strong explanatory power for depressive symptom scores and cognitive function scores.Conclusion The power of slow QEEG waves in CSVD patients is positively correlated with cognitive impairment and depressive symptoms.
