摘要
癌因性疲乏(cancer-related fatigue,CRF)严重影响了患者的生活质量,然而目前缺少有效的治疗方法。林洪生教授认为CRF以本虚标实为核心,病机涉及正气亏虚、气血失和、癌毒内蕴、脏腑虚衰,且虚、瘀、毒、损相互夹杂,她提倡将“固本”贯穿于CRF治疗的始终,灵活应用补、调、和、益、养五法顾护脾肾,充养气血阴阳;同时兼顾清源,巧用清、消二法清除癌毒、痰瘀等实邪,阻断邪正互伤恶性循环。本文将林洪生教授“固本清源”辨治CRF的经验进行归纳总结,以期为临床治疗提供参考。
Cancer-related fatigue(CRF)severely affects the quality of life of patients,yet effective treatment methods are currently lacking.Professor Lin Hongsheng believes that CRF is characterized by“deficiency of the root and excess of the branch”in its pathogenesis,involving deficiency of vital qi,imbalance of qi and blood,accumulation of cancer toxins,and deficiency of the internal organs.Pathological factors such as deficiency,stasis,toxin,and damage intermingle.She advocates for the principle of“tonifying the foundation”throughout the treatment of CRF,flexibly applying the five methods of tonification,adjustment,harmonization,benefit,and nourishment to address the deficiency states of spleen and kidney,as well as qi,blood,yin,and yang in the body.At the same time,it is important to clear the source,skillfully using the methods of clearing and resolving to eliminate excess pathogens like cancer toxin,phlegm,and blood stasis,interrupting the vicious cycle of mutual injury between vital qi and pathogens.This article summarizes Lin Hongsheng’s experience in strengthening the foundation and clearing the source for the differentiation and treatment of CRF,aiming to provide a reference for clinical treatment.
