摘要
目的了解某院国家医保谈判药品(简称国谈药品)“双通道”政策的实施效果,以及实施过程中存在的问题,为其流程优化提供参考。方法梳理该院国谈药品“双通道”政策的实施流程,统计并分析2020年10月至2023年12月“双通道”药品外购处方流转情况及咨询问题,以及“双通道”药品品规、使用科室、购买定点药店的实际情况及存在问题,以了解“双通道”政策在该院的实施效果。结果2020年10月至2023年12月,“双通道”药品外购处方量逐年增长,2023年的外购处方量较2022年增长了3.62倍;2020年至2023年的“双通道”药品使用品规数及使用科室逐年增多,使用品规数由2020年的6种增至2023年的185种,使用科室由2020年的3个增至2023年的26个;咨询问题主要为处方审核流程(100.00%),其次为购买定点药店地点(96.40%);重庆市的“双通道”药品购买定点药店数量也由2020年的3家增至2021年的87家(此后无更新),覆盖范围扩大。结论随着国谈药品“双通道”政策实施时间的延长,临床用药科室和药品品规均增多,惠及患者大幅增加。但“双通道”政策的实施过程中还存在患者对政策不熟悉、药品供应信息不对称等问题,建议采用信息化手段优化“双通道”药品处方审核和流转流程,提高国谈药品的可及性。
Objective To understand the implementation effect of the"dual channel"policy for national negotiation drugs in a hospital and the problems in the implementation process,and to provide a reference for optimizing the process.Methods Through sorting out the implementation process of the"dual-channel"policy of national negotiation drugs in the hospital,the circulation of prescription and consultation problems of"dual-channel"drugs from October 2020 to December 2023,and the actual situation and existing problems of the"dual channel"drug specifications,departments that used these drugs,and designated pharmacies were analyzed to understand the implementation effect of the"dual-channel"policy in the hospital.Results From October 2020 to December 2023,the number of outsourced prescriptions for"dual channel"drugs has been increasing year by year,and the number of outsourced prescriptions in 2023 has increased by 3.62 times compared to 2022.The number of"dual channel"drug specifications and departments that used these drugs from 2020 to 2023 has been increasing year by year,with the number of specifications used increasing from six kinds in 2020 to 185 kinds in 2023,and the number of departments that used these drugs increasing from 3 in 2020 to 26 in 2023.The main consultation questions were about the prescription review process(100.00%),followed by the address of designated pharmacies(96.40%).The number of designated pharmacies for purchasing"dual channel"drugs in Chongqing has also increased from 3 in 2020 to 87 in 2021(with no updates since then),and the scope has expanded.Conclusion With the extension of the implementation time of the"dual channel"policy for national negotiation drugs,the number of clinical departments that used these drugs and drug specifications has increased,and the benefit of patients has increased significantly.However,there are still some problems in the implementation process of the“dual channel”policy,such as patients′unfamiliarity with the policy and information asymmetry in drug supply.It is recommended to use information technology to optimize the prescription review and circulation process of"dual channel"drugs,and improve the accessibility of national negotiated drugs.
