吸入性乙酰半胱氨酸联合依替米星对肺炎患者肺功能及血清MyD88、sTREM-1水平的影响

摘要 目的 观察吸入用乙酰半胱氨酸联合依替米星治疗肺炎患者的临床疗效。方法 回顾性选取2021年1月至2022年12月在新乡医学院第一附属医院收治的83例肺炎患者作为观察对象,按照治疗方法分为对照组(41例)、联合组(42例)。两组均接受常规治疗,在此基础上对照组接受依替米星治疗,联合组接受依替米星联合吸入用乙酰半胱氨酸治疗,两组持续治疗1周。对比两组临床疗效、肺功能[最高呼气峰流速(PEF)、最大通气量(MVV)、用力肺活量(FVC)、一氧化碳弥散量(DLco)]、血气指标[血氧饱和度(SaO_(2))、碳酸氢根(HCO_(3)^(-))、动脉二氧化碳分压(PaCO_(2))、动脉血氧分压(PaO_(2))]、Toll样受体4(TLR4)/髓样分化蛋白88(MyD88)/核因子κB(NF-κB)信号通路指标、炎症因子[C反应蛋白(CRP)、可溶性髓样细胞触发受体-1(sTREM-1)、肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)]及不良反应。结果 治疗后,联合组总有效率高于对照组(P<0.05),PEF、DLco、FVC、MVV、PaO_(2)、HCO_(3)^(-)、SaO_(2)高于对照组,PaCO_(2)低于对照组(P<0.05),TLR4、MyD88、NF-κB蛋白表达低于对照组(P<0.05),sTREM-1、CRP、TNF-α、IL-6水平低于对照组(P<0.05)。两组不良反应发生率差异无统计学意义(P>0.05)。结论 吸入用乙酰半胱氨酸联合依替米星可改善肺炎患者肺功能和血气指标,并减轻患者炎症反应。 Objective To observe the clinical effect of inhaled acetylcysteine combined with etimicin in the treatment of pneumonia.Methods A total of 83 patients with pneumonia admitted to the First Affiliated Hospital of Xinxiang Medical University from January 2021 to December 2022 were retrospectively selected as observation objects and divided into control group(41 cases)and combination group(42 cases)according to treatment methods.Both groups received conventional treatment,on the basis of which the control group was given etimicin,and the combination group was given etimicin combined with inhaled acetylcysteine.The two groups received continuous treatment for one week.The clinical efficacy,pulmonary function[maximum expiratory peak flow rate(PEF),maximum ventilatory volume(MVV),forced vital capacity(FVC),carbon monoxide dispersion(DLco)],blood gas index[blood oxygen saturation(SaO_(2)),bicarbonate(HCO_(3)^(-)),arterial partial pressure of carbon dioxide(PaCO_(2)),arterial partial pressure of oxygen(PaO_(2))],Toll-like receptor 4(TLR4)/myeloid differentiation protein 88(MyD88)/nuclear factorκB(NF-κB)signaling pathway indicators,inflammatory factors[C-reactive protein(CRP),soluble myeloid cell trigger receptor-1(sTREM-1),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)]and An adverse reaction were compared between the two groups.Results After treatment,the total effective rate of the combination group was higher than that of the control group(P<0.05),PEF,DLco,FVC,MVV,PaO_(2),HCO 3-,SaO_(2) were higher than those of the control group,PaCO_(2) was lower than that of the control group(P<0.05),TLR4,MyD88,NF-κB protein expression was lower than that of the control group(P<0.05),sTREM-1,CRP,TNF-α,IL-6 levels were lower than those of the control group(P<0.05).There was no statistically difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Inhaled acetylcysteine combined with etimicin can improve lung function and blood gas indexes and reduce inflammation in patients with pneumonia.
