摘要
糖尿病、高脂血症等代谢危险因素可引起体内糖脂代谢异常,细胞能量代谢失调,出现线粒体功能障碍,细胞内毒性物质不断累积,逐渐造成心、肾组织细胞损伤。此过程与中满内热致气络受损,气化功能失常,日久形成癥瘕,最终发展至心络肾络病变的病机变化具有相似之处。膏脂不归正化,酿生浊毒,痰热瘀虚交织,内袭心肾之络,“微型癥瘕”即为能量异常代谢中所产生的“废物”。钠-葡萄糖共转运蛋白2抑制剂(sodium-glucose co-transporter 2 inhibitor,SGLT2i)在代谢性疾病中的心肾保护作用毋庸置疑,其调节能量代谢的作用途径与“清热消癥”法内涵相似。因此,能量代谢异常为“心肾络病”之进展因素,在心-肾-代谢综合征病程中起重要作用。
Metabolic risk factors such as diabetes mellitus and hyperlipidemia can cause abnormal glucose and lipid metabolism in the body,imbalance of cellular energy metabolism,mitochondrial dysfunction,and continuous accumulation of intracellular toxic substances,gradually causing cell damage to heart and kidney tissues.This process is similar to another one:the pathogenesis of the damage caused by the internal Heat due to abdominal fullness and distention,then the dysfunction of Qi Hua,and then the formation of a accumulations over time,and finally the development of the Heart and Kidney collateral lesions.The fat cannot metabolize normally,causing Turbidity and Toxin,Phlegm,Heat and stasis intertwined,all of which attack internally on the Heart and Kidney collaterals.The accumulations are just the"waste"produced in the abnormal metabolism of energy.There is no doubt that sodium-glucose co-transporter 2 inhibitor(SGLT2i)has a cardiorenal protective effect in metabolic diseases,and its mode of action in regulating energy metabolism is similar to that of the"clearing Heat and eliminating accumulation"method.Therefore,abnormal energy metabolism is a progressive factor of"cardiovascular-kidney-metabolic syndrome"and plays an important role during CKM syndrome.
