摘要
目的探讨骨折患者的同型半胱氨酸(Hcy)水平与深静脉血栓(DVT)发生的相关性及其诊断价值。方法回顾性纳入2023年1月至2024年7月在本院收治的618例下肢骨折的患者为研究对象,收集患者一般临床资料和血液学指标,根据术前超声筛查,分为DVT组173例和非DVT组445例。应用SPSS 29.0软件进行分析,绘制受试者工作特征曲线(ROC),分析Hcy在深静脉血栓形成中的诊断效能。结果①与非DVT组相比,DVT组的Hcy、D-D、年龄、RDW-SD、RDW-CV明显升高;HDL-C、ApoA-1、sdLDL-C、C1q、PDW水平降低,差异有统计学意义(P<0.05);②DVT与年龄、Hcy、D-D、RDW-SD、RDW-CV呈正相关,与HDL-C、ApoA-1、sdLDL-C、C1q、PDW呈负相关;③回归分析显示,年龄、Hcy、D-D与发生DVT形成独立相关(P<0.001);④ROC曲线结果显示,Hcy的AUC为0.783,联合年龄、Hcy与D-D检测的AUC为0.925,灵敏度89.8%,特异性84.1%,价值更高。结论术前Hcy水平升高与DVT形成密切相关,联合D-D与年龄检测可提高DVT早期预测效能,为临床干预提供参考依据。
Objective To investigate the correlation between homocysteine(Hcy)levels and deep vein thrombosis(DVT)in fractured patients and its diagnostic value.Methods We retrospectively included 618 patients with lower limb fractures admitted to our hospital from January 2023 to July 2024 as the research subjects.We then collected general clinical data and hematological indicators of these patients.Based on preoperative ultrasound screening results,we divided all patients into the DVT group with 173 cases and the non-DVT group with 445 cases.SPSS 29.0 software was used for analysis.Receiver operating characteristic curves(ROC)was plotted to analyze the diagnostic efficacy of Hcy in deep vein thrombosis.Results①Compared with the non-DVT group,the DVT group showed significantly higher levels of Hcy,D-D,age,RDW-SD,and RDW-CV,while levels of HDL-C,ApoA-1,sdLDL-C,C1q and PDW were significantly decreased(P<0.05).②DVT was positively correlated with age,Hcy,D-D,RDW-SD and RDW-CV,while negatively correlated with HDL-C,ApoA-1,sdLDL-C,C1q and PDW.③Regression analysis showed that age,Hcy,and D-D were independently correlated with the occurrence of DVT formation(P<0.001).④The area under the curve(AUC)of Hcy was 0.783,while AUC for the combined of age,Hcy and D-D was 0.925,with a sensitivity of 89.8%and a specificity of 84.1%,which was of higher value.Conclusion The increase in preoperative Hcy levels is closely related to the formation of DVT.The combination of D-D and age can improve the early prediction efficiency of DVT and provide reference for clinical intervention.
