摘要
基于“玄府-气液-络脉”理论探讨冠心病合并2型糖尿病的中医辨治思路。基于文献及临床病例研究,认为冠心病合并2型糖尿病的病机与“玄府-气液-络脉”理论密切相关,并将其病机演变规律初步概括为三期:初期玄府郁闭、开阖失司;中期痰瘀互结、气液阻络;后期阳气不振、络脉虚损。治疗上应分期论治,初期开玄解郁,辛散宣通;中期逐瘀化痰,气液流行;后期温阳补虚,络脉以荣,为冠心病合并2型糖尿病的中医辨治提供新思路。
Based on the theory of"sweat pores-qi and fluids-collaterals",this study discusses the TCM syndrome differentiation and treatment for coronary heart disease complicated with type 2 diabetes mellitus.Based on the ancient and modern literature and clinical case studies,it is believed that the pathogenesis of coronary heart disease complicated with type 2 diabetes mellitus is closely related to the theory of"sweat pores-qi and fluids-collaterals",and its evolution can simply be summarized as undergoing three phases:Its early stage is characterized by depression and closure of sweat pores,and failure of their opening and closing functions,while its middle stage is characterized by intermingled phlegm and stasis,qi and fluids obstructing collaterals,and its advanced stage by yang qi deficiency and collateral damage.It is advised that coronary heart disease complicated with type 2 diabetes mellitus should be treated by stages,with opening up sweat pores and relieving depression,and dispersing them with pungent herbs for the early stage,while with expelling blood stasis and resolving phlegm to promote qi andfluid movement for the middle stage,and with warming yang and tonifying deficiency,vitalizing collaterals and arteries for its later stage.It is expected that the argument in this study may serve to provide a new idea for the syndrome differentiation and treatment of coronary heart disease complicated with type 2 diabetes mellitus in TCM.
