摘要
目的观察针灸对体外受精-胚胎移植反复失败患者子宫内膜血流情况及妊娠率的影响。方法选取100例接受反复体外受精-胚胎移植失败患者,按随机数表法分为中医组(50例)和常规组(50例)。常规组仅予普通雌激素治疗,中医组在常规组的基础上联合针灸治疗。观察两组治疗前后子宫内膜厚度、子宫内动脉搏动指数(pulsatility index,PI)、收缩期峰值与舒张期末期流速比值、血流阻力指数(resisitance index,RI)、中医证候积分、抑郁自评量表(self-rating depression scale,SDS)和焦虑自评量表(self-rating anxiety scale,SAS)评分的变化,比较两组治疗后的胚胎种植率、妊娠率以及临床疗效。结果治疗后,两组子宫内膜厚度大于治疗前(P<0.05),且中医组大于常规组(P<0.05);两组中医证候积分、SDS评分、SAS评分、子宫动脉PI、收缩期峰值与舒张期末期流速比值、RI均低于治疗前(P<0.05),且中医组均低于常规组(P<0.05)。中医组胚胎种植率、妊娠率以及总有效率均高于常规组(P<0.05)。结论在雌激素治疗基础上,针灸能增加反复体外受精-胚胎移植失败患者的子宫内膜厚度,改善子宫内膜血流,促进胚胎着床,提高妊娠率,缓解焦虑和抑郁情绪。
Objective To observe the effect of acupuncture on endometrial blood flow and pregnancy rate in patients with repeated in vitro fertilization-embryo transfer(IVF-ET)failure.Method One hundred patients with repeated IVF-ET failure were randomly divided into a traditional Chinese medicine(TCM)group(50 cases)and a conventional group(50 cases)according to a random number table.The conventional group received standard estrogen therapy alone,while the TCM group received additional acupuncture treatment.Endometrial thickness,uterine artery pulsatility index(PI),systolic/diastolic ratio(S/D),resistance index(RI),TCM syndrome scores,self-rating depression scale(SDS)and self-rating anxiety scale(SAS)scores were observed before and after treatment.Embryo implantation rate,pregnancy rate and clinical efficacy were compared between the two groups.Result After treatment,endometrial thickness increased in both groups(P<0.05),with greater improvement in the TCM group(P<0.05).TCM syndrome scores,SDS and SAS scores,uterine artery PI,S/D ratio and RI decreased in both groups(P<0.05),with lower values in the TCM group(P<0.05).The TCM group showed higher embryo implantation rate,pregnancy rate and total effective rate than the conventional group(P<0.05).Conclusion Based on estrogen therapy,acupuncture can increase endometrial thickness,improve endometrial blood flow,promote embryo implantation,increase pregnancy rate,and relieve anxiety and depression in patients with repeated in vitro IVF-ET failure.
