摘要
胃低级别上皮内瘤变(LGIN)为临床慢性病、多发病,病理表现为腺体萎缩及黏膜肌层增厚,在其基础上伴发轻、中度异型增生。随着异型增生程度增加,其癌变程度也相应增加。因此,LGIN作为治疗的关键节点,受到广泛重视。西医学治疗本病以缓解症状为主,对于慢性病程患者疗效欠佳,并常出现腹泻、便秘等一系列不良反应。中医药治疗本病具有独特优势,通过中药汤剂、针灸、贴敷等方式,调控多种信号通路从而治疗本病。目前国内研究多以中医药治疗LGIN的临床疗效为主,缺少总结调控LGIN多种信号通路。故现综述SHH、NF-κB、PI3K/Akt、MAPK、Hippo、JAK/STAT信号通路与LGIN相关性及中医药治疗现状,以期为LGIN的治疗提供新的思路与理论依据。
Low-grade intraepithelial neoplasia(LGIN)of the stomach is a clinical chronic and common disease,characterized by glandular atrophy and thickening of the mucosal muscle layer,with accompanying mild to moderate atypical hyperplasia.As the degree of atypical hyperplasia increases,the risk of malignant transformation also rises.Therefore,LGIN has gained widespread attention as a key target for research and treatment.Modern medical treatments focus on symptom relief,but they are often ineffective for patients with chronic conditions and are associated with side effects such as diarrhea and constipation.Traditional Chinese Medicine(TCM)offers unique advantages in treating this condition,utilizing methods such as herbal decoctions,acupuncture,and topical applications to regulate various signaling pathways.While current domestic studies primarily focus on the clinical efficacy of TCM in treating LGIN,there is a lack of comprehensive documentation on how TCM regulates the various signaling pathways involved in the pathogenesis of LGIN.This review aims to summarize the relationships between the SHH,NF-κB,PI3K/Akt,MAPK,Hippo,and JAK/STAT signaling pathways and LGIN,and discusses the current status of TCM treatment for this condition.By elucidating the specific mechanisms,this review provides new perspectives and theoretical support for the treatment of LGIN.
