摘要
目的 探究锐性VS钝性腹腔镜微创剥离术治疗子宫内膜异位症(EMT)的效果及对其性激素水平的影响。方法 选取2022年01月至2024年01月期间我院收治的88例EMT患者,按不同的手术方法分为锐性组和钝性组(n=44)。其中锐性组采用腹腔镜下改良锐性分离法,钝性组采用腹腔镜下传统钝性撕拉法。评估两组不同位置卵巢皮质剥离率、剥离厚度、性激素水平、卵巢储备功能指标、卵巢体积和窦卵泡计数。结果 锐性组在卵巢门处的卵巢皮质剥离率低于钝性组(P<0.05);锐性组在卵巢门、卵巢中间的卵巢皮质剥厚度均小于锐性组(P<0.05);术前术后两组患者的激素水平对比均无明显差异(P>0.05);术后两组患者的抑制素B及抗苗勒氏管激素水平均低于术前(P<0.05),但术后锐性组抑制素B及抗苗勒氏管激素水平均高于钝性组(P<0.05);术后锐性组的窦卵泡计数多于钝性组,卵巢体积大于钝性组(P<0.05)。结论 相较于腹腔镜下传统钝性撕拉法,对于EMT患者来说,采用腹腔镜下改良锐性分离法具有较佳的效果,可保护卵巢功能,具有独特的优势。
Objective To investigate the effect of sharp versus blunt laparoscopic minimally invasive exfoliation on endometriosis(EMT)and its effect on sex hormone levels.Methods Eighty-eight patients with EMT admitted to our hospital from January 2022 to January 2024 were selected and divided into acute group and blunt group according to different surgical methods(n=44).The sharp group was treated with improved sharp separation under laparoscopy,and the blunt group was treated with traditional blunt tearing under laparoscopy.The peel rate,peel thickness,sex hormone level,ovarian reserve function index,ovarian volume and sinus follicle count of the two groups were evaluated.Results The peeling rate of ovarian cortex in the acute group was lower than that in the blunt group(P<0.05).The skin thickness of the ovarian cortex in the ovarian hilum and in the middle of the ovary in the acute group was lower than that in the acute group(P<0.05).There was no significant difference in the hormone levels between the two groups before and after surgery(P>0.05).After surgery,the levels of statin B and anti-Mullerian hormone in 2 groups were lower than before surgery(P<0.05),but the levels of statin B and anti-Mullerian hormone in sharp group were higher than those in blunt group(P<0.05).The number of sinus follicles in acute group was higher than that in blunt group,and the volume of ovary was higher than that in blunt group(P<0.05).Conclusion Compared with the traditional blunt tearing procedure under laparoscopy,the modified sharp separation method under laparoscopy has a better effect for patients with EMT and can protect ovarian function,which has a unique advantage.
