哮喘精准分型与前沿生物治疗:从表型到内型不断革新

摘要 哮喘是一种常见的呼吸系统疾病,其分型方法已从传统的基于可观察特征的表型分型转变为基于分子生物学特征的内型分型,这标志着哮喘诊治正朝着精准化和个体化方向发展。目前,高T2型(Type 2-high)哮喘的研究已较为深入,相关生物制剂的研发已取得了显著进展。然而,低T2型(Type 2-low)哮喘的分子生物学特征尚未完全明确。本文回顾了哮喘表型和内型的特征,阐述了相关细胞因子及作用机制,并总结了当前用于哮喘治疗的生物制剂的疗效和适应证。研究结果表明,在重症哮喘的诊治中,明确分子生物学机制并实施针对性靶向治疗可取得良好疗效,为后续哮喘内型的深入研究和生物制剂的应用提供了理论基础。 Asthma is a common respiratory disease,and its typing method has changed from the traditional phenotyping based on observable features to the endotyping based on molecular biological features,which indicates that the diagnosis and treatment of asthma is developing towards precision and individualized.At present,the research of Type 2-high asthma has been more in-depth,and the research and development of related biologics has made remarkable progress.However,the molecular biology of Type 2-low asthma is not fully understood.This article reviews the phenotype and endotypic characteristic of asthma,describes the relevant cytokines and their mechanisms of action,and summarizes the efficacy and indications of current biologics for asthma treatment.The results show that in the diagnosis and treatment of severe asthma,a clear molecular biological mechanism and the implementation of targeted therapy can achieve good curative effect,which provides a theoretical basis for the further study of asthma endotype and the application of biological agents.
