生物反馈疗法联合盆底肌训练治疗经尿道前列腺电切术后尿失禁的效果观察 被引量:1

Efficacy of Biofeedback Therapy Combined with Pelvic Floor Muscle Training in Urinary Incontinence Following Transurethral Resection of the Prostate

摘要 目的探究生物反馈疗法联合盆底肌训练治疗经尿道前列腺电切术后尿失禁的效果及其可能的作用机制。方法选取2023年6月至2024年5月因尿道前列腺电切术后尿失禁在南昌大学第二附属医院泌尿外科住院的患者160例,通过数字随机法将患者分为对照组,生物反馈疗法(BFT)组,盆底肌训练(PFMT)组,以及生物反馈疗法联合盆底肌训练(BFT+PFMT)组,每组各40例。对照组患者给予术后常规护理,BFT组患者在术后常规护理的基础上使用生物刺激反馈仪在患者双侧腹股沟区内侧进行电反馈治疗,PFMT组患者在术后常规护理的基础上进行Kegel训练法治疗,BFT+PFMT组患者使用Kegel训练法联合生物反馈仪治疗。观察对比4组患者的漏尿量变化、国际尿失禁咨询委员会尿失禁问卷表简表(ICIQ-SF)得分并比较各组的治疗效果。结果经过治疗后,BFT组、PFMT组和BFT+PFMT组3组患者的漏尿量均显著少于对照组,BFT+PFMT组患者的漏尿量显著少于BFT组、PFMT组(P<0.05)。BFT组、PFMT组的ICIQ-SF评分低于对照组,BFT+PFMT组的ICIQ-SF评分低于BFT组、PFMT组(P<0.05)。治疗后BFT组与PFMT组在漏尿量和ICIQ-SF评分的比较P值分别为0.4419,0.8336,差异无统计学意义。在治疗效果方面,对照组的有效率是50.00%,BFT组的有效率是77.50%,PFMT组的有效率是75.00%,BFT+PFMT组的有效率是95.00%。结论生物反馈疗法和盆底肌训练都能够减少尿道前列腺电切术后尿失禁患者的漏尿量,改善尿失禁症状,且两者联合治疗效果更佳,值得临床推广。 Objective To investigate the efficacy and potential mechanism of biofeedback therapy combined with pelvic floor muscle training in the treatment of urinary incontinence following transurethral resection of the prostate(TURP).Methods A total of 160 patients who were admitted to the Department of Urology at the Second Affiliated Hospital of Nanchang University for urinary incontinence following TURP from June 2023 to May 2024 were enrolled and allocated via a random number table method into 4 groups:control group,biofeedback therapy group(BFT),pelvic floor muscle training group(PFMT),and combination group(BFT+PFMT),with 40 patients each.Patients in the control group received postoperative routine care,whereas the BFT group underwent electrical feedback therapy using a biofeedback stimulator applied to the medial inguinal regions bilaterally,in addition to routine postoperative care.The PFMT group performed Kegel exercises as an adjunct to standard postoperative care,while the combination group(BFT+PFMT)received integrated therapy consisting of both Kegel exercises and biofeedback stimulation.Changes in urine leakage volume,scores on the International Consultation on Incontinence Questionnaire-Short Form(ICIQ-SF),and comparative therapeutic efficacy were systematically analyzed across all 4 groups.Results After treatment,urine leakage volume was significantly reduced in the BFT,PFMT,and BFT+PFMT groups compared to the control group.The BFT+PFMT group demonstrated further reductions in urine leakage volume relative to both the BFT and PFMT groups(P<0.05).ICIQ-SF scores in the BFT and PFMT groups were lower than those in the control group,while the BFT+PFMT group exhibited even lower scores compared to the BFT and PFMT groups(P<0.05).No statistically significant differences were observed in urine leakage volume(P=0.4419)or ICIQ-SF scores(P=0.8336)between the BFT and PFMT groups post-treatment.Regarding therapeutic efficacy,response rates were 50.00%in the control group,77.50%in the BFT group,75.00%in the PFMT group,and 95.00%in the BFT+PFMT group.Conclusion Biofeedback therapy and pelvic floor muscle training may both contribute to reducing urine leakage volume and improving urinary incontinence symptoms in patients following TURP.The combination of these interventions can demonstrate enhanced therapeutic outcomes compared to individual modalities,which may warrant clinical consideration.
