盆底超声及盆底肌电生理检测在产后压力性尿失禁中的评估价值

Evaluation value of pelvic floor ultrasonography and electrophysiological detection of pelvic floor muscle in postpartum stress urinary incontinence

摘要 目的探究在产后压力性尿失禁(SUI)评估中盆底超声及盆底肌电生理检测的应用价值。方法收集南华大学附属长沙中心医院2022年1月至2024年2月收治的99例产后42 d复查产妇为研究对象,以其是否发生产后SUI分为观察组(发生尿失禁,31例)和对照组(无尿失禁,68例)。两组患者均进行盆底超声、盆底肌电生理检测,及国际尿失禁咨询委员会尿失禁问卷(ICIQ-SF)调查。比较两组ICIQ-SF评分,及两组患者在静息和最大Valsalva状态下盆底超声显示膀胱尿道后角(PUA)、尿道倾斜角(UTA)、膀胱位置(BDP)、膀胱颈位置(BNP)、膀胱颈移动度(BND)和肛提肌裂孔面积,盆底肌Ⅰ类及Ⅱ类肌纤维收缩期电位均值及收缩期时间。分析盆底超声、盆底肌电生理检测参数与ICIQ-SF评分之间的相关性。结果观察组ICIQ-SF评分高于对照组[(7.9±1.6)比(2.6±0.6),P<0.05]。观察组静息状态下PUA、UTA、肛提肌裂孔面积及最大Valsalva状态下PUA、UTA、BND、肛提肌裂孔面积均高于对照组(P<0.05),最大Valsalva状态下BDP、BNP水平低于对照组(P<0.001)。观察组Ⅱ类肌纤维收缩期电位低于对照组,收缩期时间短于对照组(P<0.05)。ICIQSF评分与静息状态PUA、UTA、肛提肌裂孔面积和最大Valsalva状态PUA、UTA、BND、肛提肌裂孔面积呈正相关,与Ⅱ类肌纤维收缩期电位和时间、最大Valsalva状态BDP、BNP呈负相关(P<0.05)。结论在产后SUI中,盆底超声和盆底肌电生理检测指标均对其具有良好评估价值,可作为该病诊疗及预后的评估依据。 Objective To explore the application value of pelvic floor ultrasound and pelvic floor electrophysiological detection in the evaluation of postpartum stress urinary incontinence(SUI).Methods A total of 99 postpartum women who underwent 42 day follow-up examinations at South China University Affiliated Changsha Central Hospital from January 2022 to February 2024 were collected as the research subjects.According to whether they had postpartum SUI,they were divided into observation group(with urinary incontinence,31 cases)and control group(without urinary incontinence,68 cases).Patients underwent pelvic floor ultrasonography,electrophysiological examination of pelvic floor muscle,and ICIQ-SF survey.ICIQ-SF scores were compared between the two groups,as well as posterior urethrovesical angle(PUA),urethral tilt angle(UTA),bladder dome position(BDP),bladder neck position(BNP),bladder neck descent(BND)and area of hiatus of levator anal muscle in the two groups under resting and maximum Valsalva conditions,and mean systolic potential and systolic time of pelvic floor muscle type I and type II muscle fibers.The correlations between pelvic floor ultrasonography,pelvic floor electrophysiological detection parameters and ICIQ-SF score were analyzed.Results The ICIQ-SF score in observation group was higher than that in control group[(7.9±1.6)vs.(2.6±0.6),P<0.05].The levels of PUA,UTA,hiatus area of levator anal muscle at resting state and PUA,UTA,BND and hiatus area of levator anal muscle at maximum Valsalva state in observation group were higher than those in control group(P<0.05),while the levels of BDP and BNP at maximum Valsalva state were lower(P<0.001).The systolic potential of typeⅡmuscle fibers in observation group was lower than that in control group,and the systolic time was shorter than that in control group(P<0.05).ICIQ-SF score was positively correlated with resting state PUA,UTA,hiatus area of levator anal muscle and maximum Valsalva state PUA,UTA,BND and hiatus area of levator anal muscle,and negatively correlated with typeⅡmuscle fiber systolic potential and time,maximum Valsalva state BDP and BNP(P<0.05).Conclusion In postpartum SUI,both pelvic floor ultrasound and pelvic floor electromyography detection indicators have good evaluation value and can be used as a basis for the diagnosis,treatment,and prognosis evaluation of the disease.
